In my squad had been Garrus and Liara. I romanced Ashley (carried over from ME1, stayed faithful).

I just wanted to add that I took Kaidan on almost all my missions after he rejoined, and took Javik on very few. Garrus was with me as often as Liara and Vega were

How is that calculated?

Reloaded the 'Citadel: The Return' mission after this.
Chose Synthesis instead. Same squad.
EDI and Liara step out of the Normandy.

Ran the ending again with enough EMS to see the 'Shepard lives' snippet.
Tali and Ashley in my squad.
Chose Destroy again.
Liara and Garrus stepped out of the Normandy.

So I never had the teleporting squadmates problem, but I never ran the assault on Earth with my LI in the squad.

I'm not completely sure but I would guess it's based on the number of positive replies you give them in conversations throughout the game.  The only ending that is an exception to this is Synthesis, in which Joker and EDI will always step out.

I only took Liara in hopes of getting some heart moving final kiss or crying from Liara. No such luck. Next time she stays at the base and I take someone who doesn't die all the time.

Capitan obvious, wants to talk to you about the fan hate in regards to the endings. How did you miss it? Emotions cloud your brain?
