They don't have to be adopted. Even now, science has discovered possible ways to create eggs from male cells (so-called male eggs) etiher by replacing the nucleus of a donated egg or by growing them from stem cells of one of the males. This way, eggs could be grown with the DNA of one male partner and then be inseminated with the sperm of the other partner, resulting in children that have half of their genes respectively, like the children of a male/female couple would have. While we are only getting started nowadays and are far from practical use, they might have figured it out in the ME universe (given the time passed and the prothean technology I would be surprised if they haven't).
So Vega and MShep could actually have childern with each other (provided a surrogate mother or artifical womb is available since it seems impossible for a man to carry a child to term). The same might be true for lesbian couples as well but it currently seems a lot harder to create female sperm than to create male eggs since women lack the Y-chromosome necessary for the production of sperm. /weird science
