Today Xbox live service got screwed up, users that were online at the time got their accounts wiped and turned to 0 gamerscore and had to recover their accounts to resync them.

Users that were at the time engaged in gameplay got any unlocked achievements added to this 0 GS "Phantom" account.

It just so happened that i was in the final mission of Mass effect 2 and unlocked the following achievements totaling 290 GS

• No One Left Behind Keep your team alive through the suicide mission 75 Unlocked on 3/13/2012
• Mission Accomplished Save humanity throughout the galaxy from certain annihilation 125 Unlocked on 3/13/2012
• Against All Odds Survive suicide mission 15 Unlocked on 3/13/2012
• Suicide Mission Use the Omega 4 Relay 50 Unlocked on 3/13/2012
  
  
  
• Long Service Medal Complete Mass Effect 2 twice, or complete it once with a character imported from Mass Effect 1 75

Then i ended the game went into Mass effect 3 and checked if i could import my Mass Effect 2 character, all good no problems, then i see to my dismay that i have only 290 GS (instead of 30k).

I do some research a find out the ongoing issue, i recover my gamertag, which returned yesterday progress before i went into the omega 4 relay, so i have to redo the final mission i complete it and unlock all achievements except Long Service Medal (and Insanity run, but i was playing on hardcore).

After talking with Xbox live, live support they told me to attempt it again after clearing the system cashe, so i loaded my save from escorting Jack in the biotic barrier and finished that last bit.

Now the way i see it its not unlocked because either the tracker for the achievement never tracked the progress for the cutscene just before entering the Collector base (the one where your ship upgrades stop your crew from dieing to the collector ship) because the save i loaded after recovering the game tag was the "Restart Last Completed Mission" as opposed to a regular save (which i overwrote on the normandy in the post game where i checked my companion's reaction to our victory) or it has something to do with some flag permanently set or being unable to find my

Medal of Honor Complete Mass Effect Playthrough 100 unlocked

achievement, as you can see, that achievement doesn't show an unlock date.

This post is just for clarification on what happened, what i can do and what impacts this will have in my game and mass effect 3 (will i get the 25% extra XP in my next character?, will the Long Service Medal on mass effect 3 be unlockable, is it permanently missed, can it be manually added by contacting someone at MS ?)

you are not the only one.. I finished the game twice but the achievement has not been unlocked.
My console just froze before entering the "light" in London. I had to restart that part, but besides this, nothing else happened..
