 In ME2, I decided to romance Tali after romancing Liara in ME1. In ME3, I decided to call it off with Liara and stick with Tali, though my decision to later let the Legion upload Reaper technology to his people caused the Quarians to lose against the Geth, and in turn, I watched Tali kill herself.

Upon reaching the Illusive Man's base, Shepard is then visited by Tali in his cabin. Then the dream sequence with the kid from the beginning. (I've yet to beat the game, so I still don't know what the significance is with the little boy if there is any). After the dream sequence, Shepard wakes up in his bed with Tali sitting right next to him. Now the moments with Tali don't seem like dream sequences as well, so my only other guess is that is a huge continuity glitch.
