This isn't a bug or technical issue but more of a design issue and I guess this forum fits the issue best.
I have completed the game on Insanity, this is not a whine thread but a headsup for other Insanity players and Bioware.
When you finally return to Earth you will be descending to the surface in the shuttle, The shuttle doors open and enemies will begin firing at you, during this time there's no way to fight back in anyway, no way to take cover and squad mates are blocked from control. On any difficulty other than Insanity (maybe Hardcore) it would not be a problem but on insanity I had to do the descent 4 times before I lucked out and the enemy AI just hit me once or twice (down to one bar of health) then when I finally get to get out of the shuttle there's like 6 meters to the nearest cover and died like 6 times before I could get to it in time, so in total I died 10 cheap deaths.
I can imagine dying 20+ times before lucking out, this is in need of a fix.
