I also found the weapon modification UI to be pretty awkward. I assume this is probably a 'PC player must deal with the console-game UI' issues, but it certainly has room for improvement.
A lot of the companion conversations appear to have been 'punted'. No cutscene, just plays the dialog while you stand there. The scene with Ash hung over on the floor of the observation deck? And Tali in the lounge? The voice over alone is good, so I am sure the scenes could have been great if they'd made them.
Can't comment on the reporter myself. I told her ' no thanks' during the first conversation and never saw her again for the entire game. Not sure why she was dressed like a space prostitute?

I imagine that the statistics will show that the majority of players did the exact same thing(I know I did). As far as I'm concerned she is not a part of the game. The model looks terrible(and complete out of place with the rest of the game assests) and it just feels like a contrived attempt at adding a little "star power" to the game. Which you already have, both in 'celebrities' and vocal stars so...?

Scanning felt...off. I scan once and my stupid alertness skyrockets. Also people playing on PC have loose a split second without keybinding to select. Console players get an advantage. Yes, they are easy to outrun, but spending extra time trying to get in the area at the right angle to get some damn resource is beyond annoying.
MP having advantage with single player. You have got to be kidding me with that. Especially from what I heard, if you don't play MP and your galactic readiness was above 50%, it goes down if you dont play for a while. Seriously? I bought this game for the story. The single player. I didn't plan on touching MP for a while.
Diana Allers. Nuff said. Don't even get me started on that...please.

While I enjoyed the vast majority of the game, there were some definite moments where it felt like there could have been a LOT more. One example, specifically, would be Thessia. Considering we just went through a two to three quest series for Rannoch (depending on the choices), and all the cloak-and-dagger surrounding how we're told about the "relic", I expected a fair bit to do on Thessia. Instead, we got one of the shortest (in terms of substance) missions in the game. Palavan had a good arc, while Tuchanka and Rannoch were excellent. Then we get to the Asari homeworld and get to run down three hallways, fight KL, then leave with our tail between our legs.
Thessia felt like they were pressed for time. Some of the sidequests felt the same as well, mostly the ones in the back half of the game (and especially the last few N7 ones).
While the parts given were enjoyable, to me, they just felt somewhat rushed. As if there should have been more but was cut due to time/resource constraints.

It always decreases over a set amount of time. Hell, I started the final mission at 100% readiness and was down to 99% by the time I finished the Station and down to 98% halfway through London. Overnight, it usually drops between 3-5%. At least that's been my experience. I went a day without touching MP, and I lost 9%.

I remember the days when my Shepard didn't make gender-political comments unless I chose it. She also didn't go off on emotional tangents or presume my intentions.
But I'm willing to forgive that for an ending that makes sense.

The last part of the game which should have been the final act had no side quests at all.
The journal/quest updates were next to useless with your quests. Lost count how many times I had to do online searchs to figure out how to complete various side quests.
The game was the buggiest Mass Effect; and the pathfinding sucked. In the one game I had of it I had to reload a dozen times because Shepard would get randomly stuck on a square patch of path and wouldnt move. Could still bring up the menu and fire his gun in battle and whatnot but I couldnt move him and would die in combat.
And I didn't see the point in a fuel mechanic because they had pretty much stripped out all exploration.
Didnt like there was no mini games at all either. Nor was I pleased at the lack of a ground vehicle.
Everything else I loved, adored even. The Citadel finally felt like a large living breathing working space station, planets finally felt like big open worlds.
Fighting a reaper on the ground felt appropriately scary.
And I competely loved, loved, LOVED Kaidan.
But all those flaws I coulda completely ignored happily had the game had a sensible conclusion.

-A couple of more squad mates would have been nice.
ME2 Squad: I understand too many variables, but each one had cameos so I think it is possible to program each as a possible squad mate. What I mean is the player would get too choose one or two. Lets face it apart from Miranda and possibly Jack no one had a side story that kept them from joining you. Thane could have easily got treated by Chakwas and his room is empty, he could also die where he has to too.
Side Quests: The scan planet and find artifact got a little boring. Other than that liked side quests.
Credits: not really a detractor but I was always short on credits. Armor 50,000 credits!
NPC convos: In ME2 and ME1 we always got put into a conversation. In too many spots you stand next to npc's and pick a side. Again annoying but entirely tolerable.
-The security gate outside the waroom: I know my Shepard is sexy but come one do they really need another full body scan? Again just kind of annoying and style issue. Kind of reminds me of the elevators from ME1.
-Layout of Normandy: Granted it is largely unchanged, but I felt the Cerberus layout was better. At this point I am just nitpicking.
-Ashley being gone for a chunk of the game: Basically took away a squad mate. Yes they did it for the story but it was still too long.
You asked for grievances but just so everyone knows there are more positives.

-Far fewer conversation options. Some conversation options really didn't matter.
-That damn kid.
-War Assets were just numbers, consequences barely manifested in conversations, cinematics, etc.
-Squad members had bad timing and especially bad pathfinding AI. It should not take you a minute to get to a point I've designated right in front of me. You should not be getting stuck behind doors, leaving me to fend for myself. Tragic, because combat was otherwise fantastic.
-Choice, choice, choice. A lot of it didn't matter.
-Shepard face imports broken.
-EA servers go down for MP quite often.

- Proper dialogue reduced to Zaeed/Kasumi style monologue for most Normandy conversations
