Mako, understandable.
Planet scanning? Definitely weird.

The Crucible is a clear Deus Ex Machina. It really makes me wonder about the writing team. Bioware has always said ME was planned as a trilogy. But it seems like they were just making up stuff as they went along, since surely a weapon against the Reapers is something you'd want to foreshadow. It really took me out of the game, too. Everytime someone mentioned the Crucible, I heard "Deus Ex Machina."

Yeah i pretty much had the same feeling. The galactic tri-color deathray was meh and cliché... I was hoping for less deus ex machina and more guerilla hit-and-run ops, gathering intel about how Reapers work, gathering a fleet to retake Earth using new tactics and tech. The same way Krogan and Turians bloodied the noses of Reapers on their homeplanets.
Once the turn has tide and Shepard and co liberate Earth and kill off Harbinger, Shepard's story is over.
New games could then be made to follow a new hero, in which you start taking back the galaxy. Or even novels, or different genres of games. But the cool universe and setting we have come to love and learn so much about would still be usable for lots of things.

I forgot to mention: Shepard was arrested because of what he did in the Arrival DLC in Mass Effect 2. That's what he was on trial for. I imagine a lot of that would've been made more clear, but a large portion of the trial itself was cut from ME3.

Number 1 for me would definitely be the Quest Log issues. I basically just started exploring the 2nd time through Citadel and couldn't figure out if I had picked up quests or not.
The Miranda romance was absolute garbage too. I reloaded after it happened and went for Liara instead. That one was great.
