It seems you can do everything 'right' and it makes no difference. It appears she lives or dies at random. There are several threads if you search for "save Miranda" or "saving Miranda", etc.

maybe it’s a bug .I’ve never seen her die in three play through’s

My story arc and my character fulfills all requirements, but she still dies in my playthrough regardless of paragon or renegade, also, i never get the possibility to shoot her dad myself, no matter what.

PS: Don't cancel too early, throwing him out of the window does not necessarily mean she dies.

This is necessary for her to survive. She'll ask for access to alliance intelligence; if you say no, she will not survive.

Confirmed, I didn't do that and she died, but her dying I thought was good for the story

I just had her die after doing 'everything right', the warning, the resources, everything. It really does seem to be a bit random, or at least more to it than it seems.

Wait, she can survive? 

I got the email from her saying she wanted to meet on the Citadel but I could never find her there. I ran all around the place and she was no where to be found. 

I'm not too bothered by her death though. I never really like her. It also gave me a chance to see a drunk Tali which was possibly the greatest moment in the game.

Ditto here. And I can't find the option in Gibbed to fix this problem either.

This is yet another reason why playing games when they are just released is an exercise in frustration.

Edit: I opened the pod and didn't get the Saber. The whole mission might be bugged. I'm replaying it now.

I just redid everything from a point before the beginning on Horizon.

Miranda's last encounter is easy to miss if you are reloading from earlier save points like I do sometimes. It would be a big help if stuff like this appeared in the Journal.

I saved Miranda, but I didn't get the renegade interrupt. It makes sense. In order to build up your war assets you need more Paragon actions than Renegade. So somehow my `biotic ****`, or `biotic badass`if you prefer, has a maxed out Paragon and barely 25% Renegade. War does strange things to people....

It msut be a bug ... She was loyal in ME2, I cross her path 3 times in the citadel, used the spectre console but she dies no matter what. I load a save before horizon but she wasn't on the citadel (it's normal because i made the 3 encounter).

Any idea on how to use the gibbed save editor to check what's wrong (for instance if the "alliance access" is ok even if I did it) ?

I don't want her to die .

She has to be loyal, and you need to warn her about Kai Lang, that way she will take ''precautions'' against him, and she won't be tortured to death.

She lived on mine. Imported me2, With a Paragon shep. miranda was loyal, and i persuaded her to speak to oriana. In me3 warned her about kai leng, gave her alliance access, and spoke to her whenever able. In Horizon, chose the paragon persuade, and she lived.

does not work, i believe its the same random **** as mordin was in final mission in me2- she can just die. i meet her 2 times (missed the first time) but i warned her about Kai and gave her resources. She dies no matter what.
Trying to get plot boolean now to fix it with gibbed save editor. not sure if i success, but this really spoiled my day.

so when i first met Miranda at the Citadel i BROKE UP with her so i can have a relatioship with Ashley. then after the Citadel was atacked i red Kai Leng's file and when i talked to Miranda in the spectre's office i told her about Kai Leng and she said she would prepare for him. later when i met her in the appartmet i gave her aliance access and in the end of the Horizon Sactuary mission i managed to presuade/intimidate her father and she still died. Miranda was LOYAL AND LOVE INTEREST in my import from ME2.

Old topic but, obviously Miranda has to be ACTIVE Love Interest to survive. Since there's a chance for her to be your LI in ME3, your love saves her. (meaning she has to be LI in ME2 too)

And, there's no way to save her if you play a Female char because she can't have been a LI technically, all choices at the end leads to her death. The reputation choices implies she wasn't prepared enough for Leng because she does NOT get shot by her father. Same with non-reputation choice and shooting him. The non-reputation choice and NOT shooting him leads miranda saving her sister then getting shot and die.

I romanced her and dumped her in ME 2 for Tali and she still lived.

Miranda can survive if she isn't romanced (in ME2), regardless of gender. Guy/FemShep has nothing to do with it. Just give her Alliance resources, warn her about Leng, and P/R Henry or take the renegade interrupt. Of course like you said, if she was a pre-established romance, you must maintain it in ME3.

Steve was my LI and Miranda lived... but on my other Shepard she died (Kaidan LI) but I don't know why.

Not true at all.

My four femSheps have all saved her.

I'm a perfect 5 for 5 on saving Miranda. Whatever the variables are, I seem to have them down.

FemShep or male? Was a romance started in ME2? If it was, then it must be continued. If it's called off, she dies.

Did you warn her about Leng? Give her Alliance resources? Did you use the P/R dependent dialogue choices on Henry Lawson, or use the renegade interrupt during the conversation? IF you don't do any of those things, she dies.

The death trigger does seem to be if you break off the romance with her , she survived in my three playthroughs only one did i romance her and the guide says you can cheat or break up with her I believe or something bad will happen it doesnt spoil what

Incorrect. Mordin will have a very probability of dying revolving anything that is in direct-combat. Sending him to Escort the survivors will ensure his survival, but decrease the rest of your team.

Miranda leading first team and second team
Mordin escort
Samara bubble-duty
Jack/Tali/Insert-Random-Girl tags along with you for the final confrontation, I believe Thane works as well.

1. Meet up with Miranda for the first time
2. Meet up with Miranda at Spectre Terminal in Citadel.  Warn about Kai Leng (This might be done below?)
3. Meet up with Miranda yet again in the Appartment, give her the intel.

Also, suceed in Mirandas mission in ME2 with the "good" outcomes. (I don't know if that revolves around telling Miranda to go see her sister, but I would assume yes, since that's what I did)  <-- This might be another factor?

I also managed to obtain ~7500 EMS by doing all of this (Just to disprove the fact THAT YOU CAN indeed get the PERFECT ENDING) assuming you BOUGHT ALL THE DLC for the prior games and succeed in recruiting maximum effiency of EMS from ME3. 100% completetion, 0 Multiplayer. Perfect Ending, all survive.
