Anyone have any suggestions? I am thinking there are other missions or quests that have to be completed by a specific way. I am thinking that siding with Jack versus Miranda might be causing this or some other missions...

I didn't realize Miranda would die in ME3 until I read about it in these forums. I always talk to all the NPCs in an RPG so that I don't miss out on any side quests.

It is because you broke up with her.

If Miranda is romanced in ME2 but dumped in ME3 for another LI, she'll end up dying regardless of whether or not she was warned about Kai Leng.

Miranda is the only LI that has consequences regarding the whole love triangle thing. She can die because of it. With every other character it is pretty much a non issue.

I am really thinking it is either bugged (and BW has other priorities than fixing this one person's story) or there are other flags and quests that can cause her to live or die other than what has mentioned here.

Very old post obviously. Only replying to it because of the amount of people who still google the question. I figured out how to save her guaranteed. All the visits are required as well to the loyalty mission. But it's like the incident with Kelly Chambers, In Mass Effect 2. You have to go as soon as the Mission shows up. As soon as you leave the Asari homeworld go immediately or she'll die regardless.

As already been said, once shes loyal and given access to alliance intel she can distract kai leng with trash talk. while he defend her small penis acusations by showing her its actually big. then kai leng hears shepard shuttle and make a run for the data.
