In one of the endings, Shepard actually lives. All we need to do is look at that ending, and figure out whether he's in London or on the Citadel:

This part isn't long and not much can be seen, but you do see a ton of debris, along with some type of machine or vehicle in the background. If these debris are consistant with the assets used in London, then he's on the ground, and the whole ending has to have been an hallucination, which means none of us knows what actually happened with the fight. If this is the Citadel however, then the ending was real.

Personally, I think those slabs of cement look a lot more like the ground than the Citadel.
