So, after one of the priority missions, I went up to the flight-deck to talk to Joker, and was immediatly stapled to the deck, unable to move.  
I entered the options menu and checked my bindings, hit "restore defaults" and continued. My next step was med-bay to switch up my powers (goodbye warp ammo, hello carnage!) but the list of accumilated powers WAS EMPTY.
I was ticked, restored the save I had from returning to the Normandy... IT WAS RESET TOO.
Disgruntled but determined, I went to the Galaxy map to continue the game, scavenging for mission items and suddenly started earning credit towards an achievement I'd already unlocked!
I'll be playing through it again, but still, where's my cheevos?
