Yes, I know there's every chance this has already been listed.  But the search is giving me nothing and there's no way I'm combing through 73
pages.

So, Tuchanka, trying to reach The Shroud.  I've gone
through the ancient Krogan ruins and come out to a reaper/rachni
attack.  That's not it.  At a certain point I hopped a broken bridge. 
That's not it, either.  Further on, I died.  But when I got picked up by
the auto-save, I was returned to t he bridge, only the bridge is now
blocked, and there's nothing left for me to do but run in a circle
trying to figure out where I am.  I don't know when the bridge became
blocked, but presumably if I'm being sent back there, it shouldn't be
blocked yet.
