Anderson should not die. He's one of the people that have been through it all with you and deserves a decent ending. He shouldn't have gone how he did.

The 16 endings, assuming Indoctrination is correct and multiplayer requirements (some are tongue-in-cheek).
1) Reapers catch the Normandy during space exploration. Game Over.
2) Shepard is killed during combat at any point in the game. Game Over.
3) Marauder Shields kills Shepard. Game Over. "Perfect" ending.
4) "The Crucible has been destroyed." Game Over.
5) Shepard becomes TIM/mecha-Saren (Control); players take over playing as Shepard's LI (or Anderson if no LI) - non-human multiplayer exists for a reason; final boss is Harbinger-possessed Shepard, a la the final fight of ME1. "Bittersweet" ending; Shepard dies, but only his squad knows how far he fell; his/her legend lives on (BUY DLC!).
6) Shepard liquifies himself and merges with Harbinger, chasing Saren's ideal (Synthesis); players take over playing as Shepard's LI (or Anderson if no LI); final boss is Harbinger itself, with help from the Normandy/fleet (based on EMS). "Bittersweet" ending; Shepard dies, no one knows exactly how; his/her legend lives on.
7) Shepard awakens lying in a debris field and badly injured; he is found by his/her squadmates who use a
8) Same as 7, but 6000-6999 EMS. Good ending, but with the death of a major character (not Shepard). Requires multiplayer and the completion of a vast majority of ME1, ME2, and ME3 sidequests.
9) Same as 7, but 5000-5999 EMS. Good ending, but with the deaths of several major characters (not Shepard). Requires multiplayer and the completion of a large chunk of ME1, ME2, and ME3 sidequests.
10) Same as 7, but 4000-4999 EMS. Good ("bittersweet") ending, but Shepard dies. May require multiplayer.
11) Same as 7, but 3500-3999 EMS. Acceptable ("bittersweet") ending, but Shepard and several major characters die. No multiplayer required if you have completed near-100% playthroughs of ME1, ME2, and ME3.
12) Same as 7, but less than 3500 EMS. Acceptable ending; the Reapers are stopped, but at the cost of Shepard, his team, the Normandy, and a significant portion of the fleet. No multiplayer required; "default" ending.
13) If "Indoctrination Theory" DLC/patch is not applied: EMS <4000 and Destroy chosen; Reapers, Geth, EDI, and all synthetic life is destroyed; Shepard is killed; Normandy crashes on random Virmire-like world with remaining crew intact; Citadel and Mass Relay network are destroyed.
14) If "Indoctrination Theory" DLC/patch is not applied: EMS >4000 and Destroy chosen; Reapers, Geth, EDI, and all synthetic life is destroyed; Normandy crashes on random Virmire-like world with remaining crew intact; Citadel and Mass Relay network destroyed. Shepard survives.
15) If "Indoctrination Theory" DLC/patch is not applied: Control chosen; Shepard is killed; Reapers forced to leave; Normandy crashes on random Virmire-like world with remaining crew intact; Mass Relays depowered.
16) If "Indoctrination Theory" DLC/patch is not applied: Synthesis chosen; Shepard is killed; "Space Magic" rewrites the DNA code of all life into a techno-organic mixture; Normandy crashes on random Virmire-like world with remaining crew intact; Citadel and Mass Relay network are destroyed. Joker and EDI cuddle.

Your statement doesn't go with logic. Destroy relay will destroy whole solar system. How did Shepard breath after the entire Solar system being blowed up?

Whole ending makes no sense anyway, so what's the difference who survives?
Even if relays do not cause supernova while exploding (makes no sense due to Arrival DLC) whole Great Armada stuck in Sol system will starve to death (very positive outcome, isn't it?) .
So really no matter what do you want about the ending, no matter if You want happy or bittersweet ending You should support logical ending.
Not some cheap crap about machines made to save us form machines by killing everything that moves that make no sense, and gives You no choice, and stands agains immortal rule that made ME series so great (choice and consequence - you have no choice, there are no real consequences of what You did in ME1, ME2 or ME3 - no matter how many WA you have, or who You did ( or not) killed, saved, whatever).
For me ME universe was brilliant for being well crafted, interesting and very logical (as for fantasy universe) but I do not see any of this fundamental for me traits in the ending.

i believe an optional happy ending is a major necessity, just like in Mass Effect 2, if you didn't upgrade the Normandy right you can't escape from the exploding Collector Base, how about a ending that is determined by What you did (peace with the turians and the Krogan)(peace with the Quarians and the Geth) The War Assets brought to the fight!
Multiplayer Galactic readiness should not affect the ending ending, but as it does now, affect the Scenes you see during the initial invasion of earth.
(forcing people to pound out multiplayer for a happy ending would be a terrible failure)
