Apparently you can save Kelly by telling her to change her name, Kal'reegar's a really minor character anyway. Dunno about Morinth, never picked her. You will ALWAYS lose Thane and Legion, regardless of your choices, but you can theoretically save everyone else. As to their deaths overall, this was going to be a hard fight, and I wasn't expecting everyone to survive.
Morith gets offed in the most ugly way possible.
You find an email from her on the Monastery. She sends you another email. And then you meet her on Earth. The banshee you fight after the reaper? Has her name.
But no one even says a word, is amazed, or angry about a former squadmate beign huskified. Basically they all ignore the fact.
That was the part of the game that pissed me off by far the most.
To be fair, none of the ingame characters can actually see the names of their enemies above their heads like we can, so them not saying anything is understandable.
Jack is under a mask and they talk about her getting killed as a Phantom. Plus we know in game that a banshee is recogniseable (the PTSD soldier talking about her squadmate).
Plus considering how even the ME codex is written from the in universe point of view, oen can assume that if a name hover over someone, it's because they know who it is.

When I played through, however, everyone who could still live, did live. Mordin, Legion, and Thane passed away, but their sacrifices damn well helped save the galaxy.

Not to mention no one in ME2 knew that it was Morinth. She had simply assumed the identity of her mother Samara if you chose her. Kasumi and Kelly were the only ones to note that something is strange about "Samara" but Shepard is the only person who knows the truth.
