When Lynch didn't get his show renewed for a final season, he did bad things to everyone's favorite characters in the last episode, then ended it with the villain laughing at the audience.
I take it back. I didn't like what he did, but the way he did it was carefully thought out and very well executed. This was actually a lot worse.

The new Battlestar Galactica was pretty disappointing and retroactively ruined the whole series for me. But it was still less disappointing than Mass Effect 3, because of the roleplaying element. Everything is magnified when you're this invested in a character and their world.
Maybe I would compare my disappointment to seeing the Star Wars prequels after growing up with the original trilogy.

Matrix 3. The quite literal use of the deus ex machina, though they did the ending right.
Deus Ex Human Revolution. For a game that prides itself of having created a milestone with the first prequel, I hated those A B C style of choosing. Especially since the writers had already picked their own choice as canon, hence rendering your choice utterly unimportant.
Divinity 2 without addon. Afterwards much more satisfying.
But they were still tad better than what ME 3 did to me.

I tried to read them. The first one was ok. Then the next three followed pretty well with the story and character development. Then the last book more or less threw out eveything from the previous books. It was like the writer started over. I couldn't finish the book. I was appalled at the total waste of time I spent reading the others only to have it all changed in book 5.

Though there are very coherent analysis' about the ending, that played largely inside Shinji's head due to time and money constraints. Even attempting to formulate a coherent train of thought about the ME 3 ends up being highly speculative with almost no hints. Alternatively, watch episode 1-24 and then watch End of Evangelion instead of ep 25 & 26 to see what is actually going on around Shinji.

i loved the bittersweet ending. it was very sad plus if u didnt like it u always have ffx-2 where he lives in the endingahandsomeshark wrote...
FFX was pretty awful. I have actually not played a final fantasy game since that one.

As a fan of The Next Generation, those last two episodes were awesome. Well, apart from that horribly forced bit when the chief engineer (I forgot his name) died.
On topic, I can't really think of anything worse than how Mass Effect 3 ended. I've almost certainly seen some things that were worse, but nothing springs to mind right now.

That was the way to do sad and tragic. And Zoe kind of sets it up from the beginning during her opening monologue.
It was also supposed to be the 2nd of a trilogy, hence the kind of depressing ending, but Torquist decided to walk the MMO route like everyone else (which was, in my mind, a huge mistake). I like the /idea/ of The Secret World, but...in execution, I have this strange feeling that it's going to flop.

Farscape, before the Peacekeeper Wars mini-series. Blowing up John and Aeryn and having it end there? Really?? WTF!
Star Wars Episode III...I love it when writers want to get from point A to point 13 (the two not even fitting together) and just forces it there whether it makes sense or not.
Hmm. Oh yeah, that's what we got with ME3...
