Asking all the questions barely does anything, you just need to get him to like you enough.

Yeah, it seems really easy to get in good favors with Sten...usually every time you talk to him is approval goes up drastically. I usually find if I take him on a few quests and talk to him in camp I get. I got it in my last playthrough before I had even completed a major quest.

he does not look for a conversation with you the whole time ... no pc does that because the content is just not endless and they react on certain triggers when they come forward to you ... no trigger is also no spontaneous talk so as long as your relation is positive you have little reason to worry.
if you want to have a talk with him yourself then the classic opening for a more positive talk with Sten is the line with ... about something he said.
he does not really like to talk much by himself so you have to take the indirect approach as opener instead of the "i like to ask you a question" and give him answers in the line of a common ground with his duty/culture or if there is none available then you better give him a question for an answer to avoid a bad answer.
think to the point of making him feel good with leadership/duty in general rather than your own good or bad concepts with him.

if you can not beat him one on one then you can make him figth in his underwear. it's a cheat-exploit but if you have to do it at least it is a funny one

In practice, all I have to do is chat him up in Lothering, give him a single gift (which triggers his first stat boost), and then I don't have to do anything more than talk to him to keep his approval rising. Right now I have him at 100, and I gave him nothing more than 2 paintings, his sword, and a generic gift. I took him with me to all three personal quest stops, each time netting +2, and I (Persuade)ed him in the initial Teagan convo, as well as forcing Lloyd to fight, both of which brings positive approval.

I got all his normal conversations and like 80 approval by giving him gifts and then talking to him whenever options came up.

I found out about this duel the hard way on nightmare difficulty. Headed up into town with sten in Dragon DLC armor and myself in starter gear, only decent item i had was "the edge". Thank goodness for stun poisons.

The very first thing you need to say to him is "I'm not here to impress you." That he approves of. Give him a good painting. This game, I was able to talk to him right away after that about "what were you doing in that cage?" which will take you to his quest. It's just that it can happen that you don't get that question option.
This time, I did not get Shale until after I got the quest for Sten. Maybe it bugs Sten's quest to get Shale before you get Sten to talk? That's the only difference between the 2 games. But I did have a similar problem and it is possible that it is not the user.

I brought him to Haven once, when we arrive he question about why we were there and that was it...no duel? =(
