Close tie between the scene that followed Harby's Beam (Shep limping to the Citadel) and Anderson's death scene.
The former because it was so out of left field & literally made me sick to my stomach. The latter because...well... "You did good, son. I'm...proud of you."

1.The mission on Thessia were you lose the prothean VI and you hear people talking to Shepard and then a reaper lands and you hear screams.
2.When Mordin dies.
3.When Legion dies.
4.When Anderson dies.
5.When you stab Kai Leng.
6.When Shepard picks the choice and you see the flashback.
7.When Thane dies.
8.The beginning of the Final Battle.
9.When you pick the ending and you see what happens.

In a good way: when Tali says, "I love you" and Shepard replies, "Keelah se'lai." When you kill Kai Leng and say, "That was for Thane, you son of a ****!" Shooting Udina.

In a bad way: the ending. I brought Tali and EDI along on the final mission. When we were hit by Harbinger's beam, the first thing I thought was, "Oh god, where's Tali?" And the ending in general. Absolutely disappointing and a ******-poor excuse for an "ending." None of your choices mattered, the galaxy gets boned, the Normandy stranded and separated from Shepard.
I want my happy ending with Tali, dammit!

Tie...Tali and Mordin death scene... Tali taking off her mask gives her the edge in my book. Let me look at my home planet WITH MY OWN eyes....hooooo purrrrrrr
Thane's charge comes in close behind....
the one i thought was trippy due to what happens afterwards....was the biotic student at the academy. If you dont have Jack...the one dude that calls her the psychotic biotic is in charge. Rodriquez is being chased so Shep and dude shatter the glass....they get her and dude gets killed. The students all messed up cuz of it.
edit..forgot about the Thessia aftermath. That one asari saying the Lieutenant is dead...and then the reapers land. dayam.

Most powerful scene? Hm.. Curing the Genophage. When Wrex tells me: "Wait, I want you to know that no matter what happens, You have been a champion to the Krogan people, a friend of Clan Urdnot, and a Brother/Sister to me! To every Krogan born after this day, the name "Shepard" will mean "hero!" Now let's show them why!

I just about jumped up and cheered.
I also loved that slow, wounded stagger to the beam to enter the Citadel.
Not necessary most powerful but it definitely is my "Most satisfying moment".
How I wish I can spam the Ranagade interrupt more. That SOB clearly deserves more than a single quick stab.
I wish it was like:

Strangely enough the most powerful moment to me was when Grunt went charging into battle when I was on my first playthrough(wanted a 100% paragon one at that). I wasn't a Grunt fan at all really, but that scene with him and the Rachni had me at the edge of my seat.

I think Legion's actually. It was definitely a toss up between Anderson and Shepard sitting together at the end, Legion, and Thane.

Also the ending and the confrontation with TIM was good, but a bit of a rehash of one of the ways you can stop Saren. Still though it was pretty intense. Too bad they cut out some lines with Anderson and Shepard (I don't care that it makes the cut scene go on for too long, they were good lines.)
Also when Shepard goes up the elevator everything goes crap.

Also, finally going face-to-face with Hackett. Feels the end of an era, where you started out with this vague voice of a Rear Admiral giving you assignments, to being pretty much on a first (errr) name basis at the end of game three? Amazing.
Best happy scene would be the scene between you and Kaidan before hitting the Cerberus base.
