- Bug: Survival Ops Pack consumed, but has no effect.
Reproduce: Consume a survival ops pack in the heat of battle. Usually works fine, but a few times the pack is consumed yet health/shields aren't restored.
- Bug: Players teleporting randomly around the map.
Sometimes they appear in the floor or even thousands of feet in the air. Here's a shot a friend took from his phone during one such occasion.
Reproduce: Not sure how to reproduce this. The one time it happened to me I was downed while using a ladder, then a teammate revived me. Usually can tell it's about to happen then your movement is slow and jerky. Eventually you'll start getting stuck in doodads, then the teleporting begins. Seems to happen to vanguard players the most; perhaps it has something to do with being downed (i.e. killed) while in mid-biotic charge, or mid-ladder climb.
- Bug: Ghost player.
Player can run around unseen, untouchable, and useless. Player cannot take or do damage to enemies nor can he interact with objectives.
Reproduce: Not sure how. But to fix it, I went into cover then attempted a grab attack; suddenly the enemies realized I was there and I snapped out of ghost mode.
- Bug: AI enemies stuck in geography.
Reproduce: Triggered this on the Tuchanka co-op map; when Cerberus enemies jet-packed onto the platform in the lower courtyard, I hit one with overload causing him to fall from the map. Except the enemy didn't die, he instead wondered around below the surface of the map. When he became the "last man" standing, we couldn't hit him with anything less than a missile.
- Bug: Proximity Mines don't explode.
It seems proximity mines that weren't detonated during the round remain for the following round. However, any mines that remain effectively turn into duds. They're still visible on the terrain, but they do not detonate. Not sure if this is by design (if it is, shouldn't the mines disappear altogether) or a bug.
Reproduce: Lay some mines, then kite an enemy into a mine placed from a previous round; it wont detonate.
- Bug: Not Earning XP or Credits
After 3 successful extractions in a row everyone in our group showed to have received XP and credits. Yet every time we get back to the lobby, our XP and credits reset back 3 games. In one instance, I was able to upgrade my abilties before it took effect; but now that it rolled me back, I show that I have -5 points available.
By far the worst of the bugs we've found thus far; if it continues, we simply cannot progress in the co-op any longer.
Reproduce: No idea how to reproduce this. The only strange thing that happened the entire game was one of our group member's was disconnected in game 1, wave 3; we invited him back into the game and manage to continue just fine.
