I have finished the game on insanity without changing difficulty but no achiv gained, why?

I have imported a mass effect 2 save too.

I finished my insanity run through last night and it unlocked just fine though.

Second, first playthrough was on hard/normal.  Kept changing difficulty on the hardest fights so I could learn em for my insanity run, lol

Try just redoing the last bit maybe?

I also checked with my brother, he did insanity on his first run, and it unlocked for him.

Insanity playthrough is just playing through the entire game on insanity without changing the difficulty after you leave earth in the prologue.  It can be gotten on the first play through.

Are you talking about the Long Service Medal? That's the one that requires two playthroughs or an imported from ME2 one.

I did get the Long Service Medal on my first playthrough with a Shepard that I imported from ME2.

if you are looking for official help, you need to contact the ea-support.
Aside from telling you to make sure that you did play on Insanity difficulty without changing it once, there is not much we can do.
So far you seem to be the only one with that Problem, so ... try again? My first playthrough was on insanity und unlocked the achievement just fine.

I've talked to several more friends who've gotten the Insanity achievement without issue on their first playthrough. The only thing I can think of is that you either accidentally changed your difficulty during your playthrough at some point after the prologue on Earth, or you've got a glitch that no one else seems to have run into yet.

My recommendation is to submit a bug report to one of the devs, and try again.

I have imported the mass effect 2 save, set insanity and started the game, that's all.
