Is this an achievement/trophy? If so it is showing as 25/25 in game but it isn't in the list of achievements? Is this a glitch?

It's an avatar reward. Gives you an Omniblade for your Xbox Live Avatar.

Ok.But why is it in the section of the game where there are achievements when it isn't an achievement but the other avatar award comes from an achievement?
Makes no sense.

Because they didn't want to create a special section for non-achievements that are almost exactly like achievements save that they give you an avatar instead of points.
