Title.
Whenever I open my journal after booting up the game or going through a load screen, if the above quest is present, it's *always* highlighted even if the quest has been completed. Not a huge issue, but annoying. Shouldn't the journal default to highlighting the top entry depending on how you have the quests sorted?
Also, not a bug, but related. The journal is awful. Quest entries need to be updated depending on their level of completeness.

Yeah, this was a really annoying part of the game, both the journal not defaulting to the top of the list and the journal not telling you when you've completed a quest.

Same issue here... I'm on my 3rd play trough and this always happens. It's the same damn quest over and over. "Citadel: Improved Power Grid"; completed or incomplete doesn't matter. As said earlier, this isn't a big issue, but it's annoying as hell!
Would also like to put in my comment on the journal. Short: It sucks. I can't for the life of me figure out why they would do this to the game!? It has been perfect in the prequels to keep track of all the side quests. But now it's just a long list of stuff all stuffed together. And "more realistic" isn't the case. I would like to believe that if Shepard is so eager to travel around the galaxy scanning planets, and getting chased by reapers in space that he takes notes in his journal to remember to look for stuff he overhears random people talking about, he would also be invested enough to updated them. And whats this with having to go search everywhere all of a sudden for a "time-killer" quest thats only within the citadel!? Should have kept it the same as ME2's journal.
I hope this gets to the guy that got the idea to re-invent the standard journal system for every rpg I've played and that he sits down for an hour or so, staring in to the wall, thinking about what he's done.
