I'm not sure if I'm the only one who felt the attack of the Quarians on their Homeworld seemed a little...sudden.  Yes, there were reports of increased Geth activity, but that never translated to me that the Quarians were launching an invasion fleet.  Instead I would have though itd be better to have had a Quarian section of the gaame arrive sooner in the game - one by which Shepherd tried to stop the war from launching in the first place (though ultimately failing).  That would have been a far better introduction than the rather sudden discovery of the invasion already taking place.  If it had been introduced after the Genophage cure, but before the fall of Thessia, it would have given the story time to breath and to head towards the war section which was already included.  

But beyond that the saving of both the Geth and the Quarians could have had an impact upon the battle for Earth, reflecting in cut scenes where by a Repear bears down on the Quarian flagship, crippling it (if the Quarians are saved).  If the Geth had been saved then geth cruisers would have intervened, sacrificng themselves and giving the Quarians time to limp away and escape.  If the Quarians were saved and the Geth weren't and your military strength was low, then it would have resulted in the destruction of the Quarian flagship.  On the other hand if the military strengths were high then a third option plays out where the Repear is destroyed by trhe firepower of a dozen dreadnoughts at the last second. 

Similarly this idea could be played out for the protection of the Krogans by the Turians - if the Turian fleet wasn't strrong enough, then the Krogan trasnports would have taken a hell of a battering.  Likewise if the Turian fleet was strong enough then more Krogans made it onto the planet and the push for London was a little more succesful.  

It's little things like those that could have helped flesh out the repurcussions of the decision making we'd all gone through earlier.  Real repurcussions with potentially emotional impacts.
