Unwavering Achievement Glitched

Be sure to complete the very first Vancouver mission on insanity as well. That should unlock the Unwavering achievement.
It's kinda confusing that the Insanity achievement only requires you to change the difficulty before you finish the first Vancouver mission, but the Unvawering cheevo requires that mission to be played on insanity as well.

There are only two things i can think of that might have something to do with this.
1. On earth, I changed the difficulty from normal to narrative, played for a bit, and before anderson and i found the radio, I switched to insanity. I made sure i hadn't finished earth and never even went back into options just to make sure.
2. kai leng fight had a glitch. I had him down to just two or three bars of life left when he got me with a melee killing me. I hit "resume" and it immediately takes me to the end of fight cut scene. I thought maybe a squad mate got him just as he got me or something until i realized that all my credits and fuel were zeroed out. I only had the 55,000 for completing the mission. After completing the game, it took me back to a save i made just before that mission started. I beat leng fair and square that time, but still nothing.

I'm going to try finishing the game from my legit kai leng win to see if that will fix it. Really not looking forward to defending the missles on earth, but hoprfully it'll just be that and not yet another full playthrough.

Oh I have the same problem on PC... My unwavering, tour of duty and tourist achievments are locked... Tour of duty says that I finish only one N7 mission, even though finished all of them, unwavering locked up at 4/27... I'll try reinstall the game and see what will that do...

I completed an Insanity run, and got the Insanity achievement fine, but no Unwavering.
I'd actually kept track all the way through, and somewhere either on Tuchanka: Turian Platoon or Tuchanka: Bomb it messed up. I played those back-to-back but, when checking the achievements screen, it had me listed as 8/27 at that point where it should've been 9/27. I'd had it on Insanity from the very start and never touched it.
I kept playing, because I needed to do the others anyway and in hope it was a glitch, but by the time I finished it read 26/27.
So okay. Annoying, but I heard you could replay on Easy all the way up to the missions you needed then switch to Insanity to beat them. Replayed all the way up to Tuchanka in double-quick time. Switched it up to Insanity. Beat Turian Platoon and Bomb again. Counter still reads 26/27.
Any clues at all from anyone on this? It's the last achievement I need, and I got all of them on ME1+2 as well. The completionist in me is disappointed, heh.
