This should make everyone feel better about themselves.
I was in a quick match earlier today: Reapers, Firebase White. The four of us had never played together and no one had voice chat, but we were doing an okay job. After a bit, we made it to Wave nine. Banshees, ravagers, brutes all over the place. One of my teammates died early on to a banshee, but the rest of us managed to pull through the worst of it. We were mopping up, and I was running around looking for those last few enemies that were hiding, not really paying close attention. Eventually, I headed to the landing pad and found myself face to face with the last remaining enemies on the map: three husks.
I tried to charge, but they grabbed me and scratched me to death. Just as I went down, the team infiltrator showed up. He saw me. He saw the three husks. He made the only logical choice: instead of shooting the husks or stopping to cloak, he was going to run straight at them to try and revive me. For some reason, this bit of gallantry didn't work out, and there were two corpses lying on the ground.
So the infiltrator and I were bleeding out, watching the husks wander around. Fun stuff. Finally, our last teammate arrived on the scene. He saw the corpses. He saw the husks. So what did he do? He ran into the middle of the husks to revive us. They attacked, he went down, but not before he got the infiltrator back to his feet. The infiltrator fled off screen and I got to enjoy a brief moment of hope . . . but then, not two seconds later, I heard the announcer. "Squad member has flatlined."
I bled out, spending the last few seconds of the game staring at my teammate's corpse. Mission scrubbed; returning to base. My squad had just been completely decimated by three husks.

All my team dying in the first 20 seconds of the wave 9 against reapers on silver, soloing the whole wave and then dying to the last mob... it was a cannibal x.x

Figuring out you can't just run around meleeing with a Krogan Sentinel against the Reapers on Silver.
After dominating in bronze, I tried to melee everything in my first Silver match... I was revived 12 times.
My team mates weren't happy.

So many... for a few of examples, missing a charge with a Krogan and going from raging engine of destruction to helplessly flailing failure and getting riddled with bullets by mooks...
walking to the edge of a ledge - and suddenly sliding down a ladder into a nest of enemies...
lining up the perfect shot and being tapped on the shoulder by a rocket or phantom...
impulsively going for the revive/objective and ending up in a massive crossfire, with plenty of time to think on how I really knew better than that...

Almost had another to add to the list tonight... I was playing my Asari Vanguard and I hadn't gone down at all the entire game. Then we're in the extraction zone about 40 seconds from takeoff and a Geth Hunter uncloaks right in front of me, tearing away at my health. So I hit charge to push in to him and repair my barriers... but as I do, he cloaks and I lock on the the Pyro a few feet back, outside the LZ. I thought for sure I was done - but not so: trusty lift grenade, oh how I love you

Usually I'm totally focused on whatever I do, like reading, watching a movie and playing a game as well. This time I was about killing a banshee, wondering when to use my cooldowns, whatever, when my cat just sneezed next to me... my mouse fell off the table, so obviously banshee lady killed me.

My most embarrassing moment was in the lobby...I selected ready, was green, all good to go. Waiting for the others and for it to load I turned away for a moment and talked to my old man - fair enough usually, the sound makes it very much clear when the game starts...except something changed, when I looked back I wasn't ready anymore (that happens, i guess, when the host changes the setting for the game?) and had kept everyone waiting. That felt crappy.

I have two: my Asari Vanguard got one shotted by a Phantom. IMO, a character with Stasis should NEVER get one shotted by a Phantom. So, "that was embarrassing."
But most certainly the worst one ever was, "wirelesstkd was killed by Husk." That was embarrassing
was playing Asari Vangaurd... right at start of wave see pile of enemies coming with phantom in the lead. Decide I need to stasis bubble them all! Hit Charge instead like a noob. Phantom 1 shots me. FML.

A very nice infiltrator kept resurrecting me when I got stuck in a corner with a brute on top of me. He'd pick me up, then the brute would start a stagger chain on me and I'd be down again before I could wiggle around him and break free.
After the third time I finally was just like "dude just forget it, heh, this brute is just not going to give up on me."

10th wave against reapers on silver and we have to upload the data.
2 banshes and a brute are coming together from the same direction and i used both rockets without killing any of them. and no, banshes were not jumping.
They wiped us out 10 seconds later.

Played a bronze mission against Reapers last night. Level 11. One Banshee. I don't feel like spamming 87 incinerates into her, so I just grab my missile launcher to take her out.
Fire. Miss.

On my Asari Vanguard, firebase reactor vs. reapers.
It's wave 9 or 10 and we're playing cleanup, I'm mopping up some marauders/cannibals on the groups flank while they are handling a banshee at the landing zone. I'm down to the last marauder, he has no shields and I go in for the charge to finish him off.
But wait. . . my aim was just 1 8th of an inch over his shoulder... instead of targeting him I go zipping full speed across the map STRAIGHT into the banshee who was more than happy to give me a hug upon my arrival. Insta dead.
I tried to explain what happened over the mic but frankly I was laughing too hard. I can't help but wonder what the others where thinking at that moment.

Trying to be fancy with a missile by shooting at a big pack of enemies with heavy units mixed in(banshee, atlas, brute, geth prime, etc.) only to have it whistle away and keep flying.

Salarian Engineer: took a Brute down to 1 bar left, only a couple rounds in Pistol backing up and firing and about to throw Incinerate ("yeah I got this") to finish it off but threw out a Decoy instead. Brute charged right through Decoy and downed me.
