 1st, main issue: During disc swapping (usually when swapping to disc 2), I'll put the new disc in, but it won't read and I'll have to return to the Dashboard and restart the game. Not game breaking, thanks to the save beforehand, but EXTREMELY annoying.

2nd, minor bug: During the Rachni/Grunt mission, when Grunt responds to your second squadmate (I had EDI and Garrus the first time, and Javik and Garrus the second), when Grunt does his stoner laugh, there some kind of glitch where I see some broken part of a teammate or something. Whatever its coming from, some black triangle cuts from the bottom to top of the screen. This happened on different consoles and different TVs.
