Booted the game, and after pressing START I had a message pop up saying something about how I needed to be signed into an Origin account to be able to access the online functions of the game. I pressed A (the only button available) to proceed, and the game took me to the main menu without any further error messages.
When I got to the main menu the first thing I noticed was that there was no "resume" option at the top of the list. Instead I had to pick "Load Game" and manually choose which saved game to load up.
Then as soon as the game started I noticed that the Y-axis of my right stick wasn't inverted. Further investigation into the options found that all of my settings had been forgotten and reset to default values.
Then I shot the first enemy that I saw and it said I had progress of 1/5000 for the Veteran achievement.
I quit the game and went back to the Main Menu to check the achievement progress in the Extras menu. I found that all of my stats had been reset to 0. Even ones that I have already unlocked are still showing as being unlocked but the stat underneath says that I've done nothing to earn it.
I'm concerned that some of the achievements (such as the ones for doing a certain number of N7 missions) aren't going to be achieveable now because there aren't going to be enough of them left to do before I finish the game.
I love this game to bits, but this has really put me off bothering to finish it now.
I'm sure BioWare are already aware of the issue, but what I would like to know is what is the likelihood of my original stats being restored? or have I lost them forever?

Twice.
On my third playthrough now and I'm up to 400 kills, which is probably close to 3400 in reality. No big deal, I suppose - but having 100 % achievements in both ME1 and 2 I was kinda hoping to ace the final installment of the trilogy as well.

I've also noticed that in the accomplishments screen off the main menu "Bruiser" is showing as unlocked (kill 100 enemies with Melee attacks) but not "Striker" (kill 25 enemies with Melee attacks)... I'm assuming that's because there is no achievement associated with it. Neither of the accomplishment badges are showing up as unlocked for the DLC achievements, either, although it does still show both of those on my Xbox Live profile.
As some others have said, I love this game and will keep playing it, but something like this is a very annoying glitch, especially the one for getting 5,000 kills... that takes a lot of game time, and if these statistics are getting reset multiple times, it will make that nigh impossible for many gamers to get.

The issue seems to be that ME3 is, at random, replacing the .gpd file in your gamer profile with a fresh copy.
For those who don't know, every game you play adds a .gpd file to your gamer profile. That is why it gets larger as you play more games. This file can contain many things, such as preferences, what missions you have unlocked, etc. But it always contains the text and icons for achievements.
When the file is reset for ME3, your currently unlocked achievements, which are stored in a separate file, are preserved, but any progress you have made towards other achievements, along with all bonus powers that you have unlocked, are reset, as that data is stored solely in the .gpd file. That file also stores things like your control settings, which is why those are going back to default.
I suspect that this issue has something to do with the process of connecting to the EA servers. The login info for your EA account is also stored in the .gpd file, and it is accessed immediately when the game starts up. (side note: anyone notice how long it takes to log in to those servers? Compare that to ME2, which also connects to EA servers at launch....)
Let's just hope they really are aware of this and can patch it, otherwise I'll never get some of these achievements... For now, I have disabled to option to auto connect to the EA servers. Since doing that, I have not had any issues.

I am half-way through an Insanity playthrough, having completed three different N7 maps on Insanity.
Does anyone know if my progress towards "complete the game on Insanity" and "complete all maps on Insanity" have also been reset? Is it worth continuing the game for those Achievements or should I just abandon it for now until a fix is released?

Yeah, this just happened to me, too. I am also through my Insanity playthrough and the Unwavering progress is gone. Will it still give me the achievement anyways or should I just give up and start over when this is fixed?Stenun wrote...

From what I've seen, you should still get the achievement for beating the game on insanity, as that is stored in your save file, however you will not get the achievement for beating all missions on insanity, as that is stored in your gpd file. Easy way to check: go to extras > accomplishments from the main menu and check the counter bar for that achievement. If it dropped back to zero, you have been reset.

Someone told me it happened when loading the game, asking to connect to your Origin account. One solution seems to leave the game without acknowledging the connection to Origin (or rebooting the console), redownload your profile and then go back to Mass Effect 3 and it should be fine.
