This is a known issue. Here's a transcript of a discussion with an EA tech person.
Onkar: Thanks for contacting EA Help! My name is Onkar how may I help you?
you: XBOX 360, Mass Effect 3. Achievement progress still keeps resetting.
you: Known issue or not?
Onkar: Please allow me some time to find the information that you are looking for.
Onkar: I
can understand how disappointing it is. Some of the customers have
reported this issue. There is some update going on. Our developers are
aware of it and are working for a resolution. We appreciate your
patience while the issue is being looked into.
Onkar: Is there anything else I can help you with, *****.
you: Is this an official statement or your personal opinion?
Onkar: This is an official statement.
you: Why do not the devs post ANYTHING related to this bug on the official forums?
you: There are at least a dozen of threads discussing this particular bug.
you: Not to mention everything else.
you: No feedback whatsoever.
you: Not your problem, I know - but active feedback would make everyone's lives easier.
Onkar: I apologize for the inconvenience caused.
you: And that answered nothing.
Onkar: Thank you for bringing this to our notice. I'll definitely escalate it to the higher authorities.
you: I thought it was a known issue already.
Onkar: Yes it is.
you: So why do you need to escalate it then?
Onkar: I'm appreciating your feedback.
you: Sure.
Onkar: It's about the forum part.
you: They
read it on a daily basis - that much is known. You're probably a gamer
as well, so you must know how annoying it is to have ZERO feedback on
anything.
Onkar: I don’t blame you for being anxious, had I been in your place, I would have reacted in the same way.
you: Any kind of list of bugs/issues acknowledged so far?
you: Or a time frame for a patch?
Onkar: I'm afraid, I would not be able to give you the time frame.
Onkar: I hope it would be resolved soon.
you: Yeah, well. Unfortunately what you or I hope is completely irrelevant.
Onkar: I can understand your concern.
you: So, the bug being acknowledged being an official statement, I'm free to copy/paste this discussion to the official forums?
Onkar: Yes.
Onkar: Thank you for contacting EA. Is there anything else I can help you with today?
you: Thanks. Nothing further.
Oh wow... You're a horrible person.

Oh wow... You're a horrible person.
Feel free to think of me what you want. I was just frustrated. Frustrated because that discussion with a support person took 82 minutes. 82.
Frustrated because I spent 82 minutes of his time and mine. I had a day off, so it's not a big deal, but there probably were some people who had actually some tech issues they needed help with. All I needed was an answer to the question "known issue or not". That answer alone took 45ish minutes.
Frustrated because the devs hadn't acknowledged the issue on the forums. You know, the official BioWare forums. The official channel of notifications. Thus, wasting the support's time and mine.
Frustrated because the common practice seems to be releasing games half finished nowadays.

I called in a favor from a friend and checked out this issue on an xbox dev kit. My original theory on page 1 of this thread seems correct. The game is failing to see the existing gpd file in your profile and instead overwriting it with a new copy as if you were just playing the game for the first time. I would imagine this issue it patchable, but I would love to hear from Bioware on this issue.
I did what you initially suggested and disabled the auto sign-in to EA servers. No stats reset issues since, has been about a week now. *knock on wood*.

Hi Bioware. I do not want to play insanity if it will reset halfway through.
I bring up a good point here, waiting for Bioware on this.

Both times I have had this happen have been the next time that I have booted the game after using the ME Datapad app for the iPad. This links the app to your game using your origin ID, and the error message that crops up when the stat wipe happens is an Origin ID problem.
So, anyone else having this problem after either a) using the ME Datapad app or

When I suggested to him that there was no way that doing that would change how the game behaves in relation to saved data, and then asked him why nobody from either BioWare or EA has responded to this issue on the forums he said that it was because NOBODY HAD REPORTED IT ON THE FORUMS.
At this point I reminded him that I'd already told him about the threads that have been on here for well over a week and ended the chat.
As much as I love this game, the lack of any response from the devs or publisher means that I'm just going to go trade this in later today. Sorry, but Ive already lost at least 3000 kills towards the veteran achievement because of this bug, and i really don't fancy starting a second playthrough on insane to then lose my progress towards the "all missions completed on insane" achievement either.

Why are you treating this person with disrespect? It's not his fault.
The EA Advisors are useless. Slate me all you like but after the chat I had.
You are now ready to chat with Jagtar.
Jagtar: Thanks for contacting EA Help! My name is Jagtar how may I help you?
you: On main menu of Mass Effect 3. I dont have the option to resume my save game,
you: Also my in game stats have also reset?
you: WTF
Jagtar: Hello
Jagtar: I'm sorry for delay in response.
Jagtar: Are you trying to import save game for mass effect?
you: No. I have completed the game already and nearly have all the achievements
you: I had over 5000 kills :|
Jagtar: I'm sorry to hear that you lost the stats in the game, I would suggest to back up your profile every so often.
Jagtar: Documents > BioWare > Mass Effect 2 > BIOGame > Profile.
you: WTF!
you: How am I supposed to do that on XBOX
Jagtar: Once you have the right devices, in order to save gaming progress, you need to be on the game that you want to save in order to save the gaming progress in which you are playing at the time.
you: WHAT!!
you: That makes no sense
Jagtar: You can visit the link given below for instructions on how to save games on xbox:

you: I would like to speak to someone else!

That's just sad. They're always pretty useless though. It's like they hire people that have no clue what they're doing and get paid to give stupid responses.
It's nice to see how wonderful Bioware is by *still* not responding about this issue. They've already got my money so why do they need to keep me happy, huh?

Its silly considering one advisor said they know about the issue yet no info from Bioware. They need to be more active letting people know what they are going to do before everyone lose patience

and exactly the same bug happened to me twice for the Unwavering achievement (insanity missions reset to 0/27)
Basically I decided to give up playing after the first game because I will never be able to have 100% achievement... I'm so disappointed...

This is EA, your/our only option is to stop buying their games, force them to properly make a game and not rush the release, or go out of business.
