I am also curious as to the situation on this issue. I am missing just the veteren and all missions on insane accomplishments. of course the striker and all the dlc ones do not show as completed in the extras part of the menu either.

So my Mass Effect 3 caused my computer to crash, when it was restarted, I had the following issue. All my achievements have been reset D:

Sigh... My "From Ashes" and "Leviathan" achievements keep resetting too.
I even tried deleting everything (game and saves), then reinstalling, while downloading my saves from the cloud; and I still have the issue. So even my cloud backups are broken.
It has been half a year since this was initially reported, and still no fix.
