Sometimes when I'm having a conversation with somebody (it's rare, only happened to me 2 or 3 times) and it pauses at a prompt, after I select what I want to say the other character repeats the audio of their last statement, however the conversation continues because it then gets to the next point asking me what to say and I'm usually confused because I don't know what they really said so I just re-load so I can hear what they were really supposed to say.  I haven't been able to notice any specific circumstances under which this happens, and it's happened talking to a couple of different people in different places, so I don't really know what to tell you that would help you figure out what causes it.

I pre-ordered the game and I love it, but it's kind of a mood killer when you're really getting into what's going on and you really want to hear what they have to say, and then they have an artard moment and repeat themselves,

i had problem during bad orphanage quest in Denerim. game made me repeat some sequences few times before quest was completed
