The journal wasn't as indepth as I'd like. There could have been more conversations with squad members. Have your LI be with you in the beginning. I had a graphical glitch in the ending where Shepard's eyeball popped out of his head. They shouldn't use stock images.
Honestly I'm grasping at straws here, it would have been perfect with a good ending.

The journal system was absolutely horrendous. Also the auto-dialogue was annoying but i could live with it.

lack of quest tracking journal, as well as too many actions being prompted into [spacebar]. It's simply fricking annoying storming next past one rock and just as you're gonna pass another, your shepard takes cover, and gets blown to bits by an enemy 
They should have made it possible to seperate Storm, Take cover/evade/roll and Use Item/button etc from the spacebar.

Honestly? It'd be very minor - that I have to play MP to get enough EMS for the best ending (if there was an ending that could even be called best).
Which isn't an issue, because I'd love to play more multiplayer.

The quest journal was awful
Revealing Tali's face with a picture instead of a 3D model. It was a nice face though.
The ME 2 squad getting the shaft.
Taking back earth wasn't epic enough. It was just the same 3 man squad combat.
There was way too much auto dialogue.
Having to play multiplayer to get all the different ending variations.

Well, I'm assuming a good ending may include a face-off with harbinger, so I'll skip that one. I wish e game had more personal dialogue instead of e quick talk that we get for most of the game. You would no your crew members, especially your LI would look at you when you speak to her
Oh, and missions not updating as you progress through them is lame. I don't know how many times I ran around the entire citadel seeing If I had any artifacts to deliver since I can't tell if I found it already or not

How short it was and not seeing war assets but honestly if the ending met my expectations I would've been fine with almost any flaws... unfortunately the game was fantastic except for the ending

- Lack of locations to visit during "downtime" Citadel is cool and all but some extra places to procure weapons and quests from would be nice. Perhaps refugee camps or encampments on the locations you liberate during the course of the game.
- Speaking of quests, seemed like ME3 had a lower amount of quests. My last ME2 play through on casual, skipping all dialog and cut scenes still tooled in at 40hrs for a fully complete game. ME3 full game clocked in my first play through(on Hardcore) at 35hrs, something seems a bit off about that. Granted the story was great(discounting the ending of course), but the side missions just seemed a bit lacking. If I want fetch quests i'll pickup Skyrim again.
- Umm the F***** ME1->ME2->ME3 facial import bug. I was pissed at ME3 before I was even able to play it because I couldn't use "my" Sheppard.

Definitely the journal.
The consistent camera 360 thing that has been annoying me since release.
Some bugs during conversations, and one curious one in Huerto Memorial where I stood near the corner of a room and ended up on the ceiling... It was fun for a while.

The Journal and a few instances of lazyness on BioWare's part, but nothing game changing or breaking.
