1. Liara's friendzone talk....I get that convo makes you committed to her but it could have been written better for people who have been romancer her since the first.
2. Lip sync glitch - It only happened in Zaeed like convo's or one or two others but it should still be fixed. Shepard's mouth moved up and down and somehow formed words...wtf
3. Journal - The missions should have been separated into Priority and Assignment again and be separate from the codex.
4.Auto dialog - Hate the Zaeed like convo's with many people. I don't care if it's just one comment but I'd like to enter the cinematic convo state when I'm talking to people not the Zaeed version where I mess with stuff in his room while he's talking.
Other than that. It was near perfection other than the endings

1. Lack of bullet points in your Journal.
2. The extended scene with Anderson being cut out of the final game. (It's not as good without out.)
3. More character interactions, there was a lot of autodialogue, especially with Ashley.

The lack of a Krogan battle buddy was disconcerting to me.
I miss the LI portrait in Shepard's crib.
I would really have liked to see the Rachni contribute a lil' bit more to the story and war effort.
If Shep had a kid I think it would have been a good thing for the series. Import your save...your next shpard in ME4 coulda been Asari, Human, or an interesting mix.

Lobotomized dialogue choices
Lack of any real story branching.
People just seem to acknowledge shepard's actions rather than them playing any real part. (councilor choice ignored, collector base ignored, cure the genophage despite whatever happened to the data.) This was my primary complaint from ME2 and early on from ME3 so I was really hoping for some payoff from the ending.
Then the...incident happened.

My only complaint if the ending was fine would be how you happen to stumble upon quests not knowing who gave it to you or having to talk to the people as much like in ME1 and 2 in order to get them, and how most of them are just search missions instead of actually landing somewhere and getting things, but with the inlcusion of Multiplayer and the immersiveness of everything else about ME3, I don't mind this process for the scan quests.
