I've had this happen in several MP games now and it is very annoying.  I use Biotic charge and find my character starts to float just a few inches above the ground, the mve I try and move the higher I get until I suddenly teleport above or below the map, once there I cannot do a thing for the rest of the game, can sometimes use powers but no animation for it.

Other players in the game report seeing my character still on the map, you can occasionally get shot and go into a revivable state, get revived, if you do not get revived you do not die and just go back like you were never shot.  Other time you get shot but can never go into revival state.

I cannot be 100% certain, but it maybe linked to charging into the Cerberus guys with shields.  Has only happened to me vs Cerberus so far.

I've totally been having this issue too.
It seems to happen whenever you die during a charge. I've also noticed that after this happens, you can use all of your powers except for charge. I usually just try and shoot things until I run out of heat sinks.
It's not just a PS3 issue either; my fiance has the same problem on Xbox.

I can still use charge after it starts and I am near ground level (though it is buggy and takes several presses to activate), until it warps me really high or under map, then it cannot target an enemy I guess..

After it happens for me, for a few waves, I'll be near the ground and the game is semi-playable. After a few waves though, I either disappear under the map or far above it. The only consistent thing is that after it happens, I cannot use charge.

its a glitch in the game not just for vangaurds my had it happen to almost all my characters
