I bought ME3 at a midnight launch, came home, put the disc in my XBOX, and it's been there ever since. I have not touched the disc nor the system since March 6th (it has not been jostled, bumped, etc.).
Today I was playing multiplayer and after the match was over it froze on the loading screen. I left it alone for about 10 minutes just to see if it would resolve and it didn't. I was able to pull up the XBOX menu and back out to XBOX home.
I immediately tried to launch ME3 again and received the message "disc is unreadable." I removed the disc, cleaned it with a cloth, and tried again. "Disc is unreadable."
After freezing during multiplayer "disc is unreadable"

Did you clear the 360's cache and/or delete corrupted avatars/dlc?
A few people have posted that works for the disc read error
