I couldn't unlock the unwavering achievement and was looking into why this might be happening. I looked through my missions completed and realized that mission Tuchanka: Turian Platoon wasn't even listed.
I started a newgame+, completed the level a second time and after doing so the mission was gone from my journal.
Any ideas how to fix this or what is wrong?

I recorded this video to show that the mission appears before landing on Tuchanka but once I'm back on the Normandy it disappears. I'm shelving the game until the bugs are fixed. Here is a link to the video:

Anyone get round this? I have finished this mission 3 times on insane now and it still isn't showing up. This is all I need to get all the achievements.
