 Add yours here too, make it short and easy to understand.
  • On incap players can frequently fall through the map and become invincible/not able to deal damage, forcing eveyone to quite and loose everything, should they become the last one alive
  • SPACEBAR. I am sure I don't need to explain that one.
  • Phantoms are randomly and often spawning invincible to powers.
  • With latency higher than 100 projectile weapons like scorpion or cobra do not work/projectile disappears.
  • After revive powers are unavaliable for some time, which makes it impossible to, say, charge away from the hot zone.
  • Power remapping does not work.
  • When heavy melleing phantoms, they can interrupt the melle in the middle of it with their insta-kill.
  • Cobra missiles sometimes simply disappear and don;t do damage even with you hosting.
  • If someone is in a situation like in [1], but the mission is complete it takes ages to load to the results screen.

Mods, make it sticky, if you can. It is very inconvenient to search various threads on the same issue to see if people are having it too. As long as posters here are constructive and short, it should be easier for everyone to have all bugs listed in one thread

In-game freezes for 5-10 seconds when using melee attack on a mob, or using powers, when your close to the mob that is.

Phantoms in stasis most of the time become immune to melle attacks.

I'm in my third playthrough of SP on PC.  I noted two specific game "bugs" or "glitches" not sure which term applies until I explain. 

1.  During the first trip to the Citadel, where you have the option of obtaining the "Mercs" from Aria...I was able to recruit both the Blood Pack and Blue Suns; however, when I went to talk to Jona Sideris at the Presidium Commons C-Sec office, and then decided to choose her "lackey" instead...I never get the Eclipse mercs and am unable to talk to Commander Bailey to tell him that he can leave Sideris in prison.  What gives on this?

2.  When I first went to talk to Liara and she introduced me to "Glyph", Sheperd's head turns to look at Glyph, and stays in that position and doesn't turn her head back to address Liara as she is talking to her.  It is rather awkward having Shepard's head turned either completely over her shoulder either right or left, and not being able to face forward again until that sequence is over and you leave Liara's room on the Normandy.

Has anyone else experienced either of these?  Just thought I'd let the Dev's know about those two that I found specific.  Nothing else has occurred yet.  Here's hoping.

iremojllah, on 1 did you talk to the "lackey" in the refugee area in the citadel? If you haven't you need to do that to get their cooperation.

game freezes and goes non responsive as soon as i touch the ladder trying to reach roof for female krogen. game is unplayable, past this point.

Can not scan from the galaxy map if you have keymapped right mouse button to another function.

Picking up a mod that is two ranks higher or more (or any mod beyond Sur'kesh) can apparently cause a full-on game crash.... Dunno if that's a bug, or just an issue for me.

powers sometimes have no effect; warp and incinerate seem to be the worst; but i've had grenades cause no damage, tech armor and adrenaline rush fail to start up, and medigel not recover my health or revive squadmates/ revive in mp. rather annoying to have an animation but no effect.

Additional Bugs

Sound Bug:
If you play sound through 5.1 channel speakers, then switch to a bluetooth stereo headset, play for a while then turn off the headset (which should return sound to the speakers), no sound is present or sound only is present in the center channel.

Only fix is to restart the game.

Neither previous ME title had this issue.

Occurs on Windows 7 and XP (both have different bluetooh stacks)

I have had this problem as well.  I have even had the odd vanguard biotic charge do no damage or knockback   Also sometimes I have to hit the power button like 3-4 times for the power to actually activate, same thing with cover.

I noted that when I tried to jump on the moving dancers platform in Purgatory, I fell into "load game land"...

I seemed to have a lot of company there, it was like a NPC storage level.

On the bridge around 0.5 meters from Jokers seat there is a sport which makes Sheppy look wierd when he stands on it. Stand on it for more than 5-10 seconds and Sheppy will be stuck with no hope of getting out, so game has to be reloaded.

Some weapons on NG+ are not present at all. Predator does not upgrade when Anderson gives it to you in the Prologue (meaning max it can stay at is VII when upgraded instead of X), and the Widow sometimes does not pop up on Thessia, meaning that again, it is stuck at VII instead of X.

This. I was tolerant and put up with many bugs but not this, I hate being surgically careful where I step as I engage in convos with Joker and EDI. Not cool when a player has to do that.

The reload animation of the Widow doesn't alwasy play. I tested this with a human infiltrator in multiplayer.

Sometimes in a multiplayer game the shuttle doesn't leave. If this is the case, some enemies spawn above the ground and keep floating there, and players can't use ladders.

Vanguard is very buggy in multiplayer, no matter the race. It happens quite often that my vanguard gets stuck in the environment and starts losing hitpoints.

We played yesterday with friends 'n stuff. Mostly stuff though. Anyway, all of those confirmed yesterday by 4 people.

The monitors in Liara's room on the Normandy SR-2 often face a ridiculous direction depending on which way Shepard is facing (and not based on where he is standing). They'll sometimes even face towards the wall with their wall mounts poking through them. This is not a consistent error. Sometimes, they'll face directly at Shepard as he moves around. I'll assume that they're supposed to face either Shepard or Liara at all times.

In addition with a Lair of the Shadow Broker import, Shepard and Liara often try to look behind Shepard (without an Exorcist event) or look at Glyph when they are talking to each other regarding Shadow Broker events. This particular bug did not happen when I did not import a game.

I found that saving at the spot and reloading that save allows me to simply move Shepard out of the spot, but a weird animation happens at the spot when getting him unstuck after reloading. For me, he tends to get stuck there when talking to EDI.

i leveled 3 classes and promoted them with no pack or anything i have the online thing checked and nothin i tried the live chat and i got the bioware social network for the last weekend and no pack. then i live chat again too have this one tell me to post in the forums... i want my pack that i did all that work for plz! sorry dont know where to put this... i want my pack :(

I'm having problems Lesuss mission: monastery
As the elevator is stopped you have to jump the gap and down the stairs at '[and so good, but when I go down to the "second level" where the lantern light gun, the character to porpoise down the second ladder and it will it's like down, but he remains stuck in the same place and not go down that ladder. I've tried almost everything but I can not go down and the difference to my last save game is about an hour, not going out looking for artifacts on planets all over again No! wanted to know how you could help me?!

The only bug which really sucked for me is the one where you get stuck behind Joker's chair. Reloading didn't work, nothing did. You can lose a good bit of time there if you've been running around speaking to squadmates and stuff.

I have video footage of [3] occurring. So far it has happened on two occassions for me. It makes it impossible to complete the round as the final enemy cannot be killed by any means (even cobra). I was hosting on the second occasion so I was able to quit and allow host migration to occur so that the other 3 players could keep their xp/credits as the round restarted. Obviously I lost all progress. :(

I've also experienced [8].

[6] is highly irritating as you have to go to Options>Key Mappings each time you change class in order to customise the powers keys.

+ I posted it elsewhere, but I'll post it here too: The human adept shockwave power is prone to becoming bugged during matches. It ceases to knock back OR detonate warped/reaved/singularity targets after a while and simply travels through targets with no effect or impact sound effect. It will then begin working again later on in another round. I can reproduce this problem in every game I host/join, and it seems to happen after you cause a certain number of shockwave impacts - (like there's a cap on the number of shockwave impacts you can cause in a set time).
