ETA: Oh and I believe you have to choose the destroy option.

it could mean that there's a posibility that sheppard MIGHT survived, but probably bad injured.
sorry if my english is not good enough, is not my native languaje and doing my best to describe it perfectly.
PS: shouldn't this post be in Mass Effect 3 Story and Campaign Discussion board?

Supposedly you can if you have an EMS score of over 4000. I have yet to get that many war assets to bump up my score although if you play the MP it will bump it up. I've seen that it is possible on Youtube but right now it's out of my reach on the PS3 and I dislike MP, so I'm outta luck unless they adjust it.

you mean the total of war assets? ( i have the game in spanish so i'm not sure if this is what you're talking about) cause i had over 6500 of that when i beated the game, and that's what i got... that perhaps he survived with the destruction of all sintetics choice... perhaps

You can also raise the rediness rating by using the datapad app for Iphone/Ipad. No use if you don't have one but it's something

i'm not sure if this counts, but i made the 3 endings (reloading the last mission) and in the one where you destroy all sintetics, in the end it seems a crash site (i'm not sure where) where you see a body (the chest part) wearing a N7 armor suddenly recovering breath.

Congradulations you chose the correct ending. You beat the reapers, and you live. Choosing the destroy option is the ONLY way to get that "bonus" cutscene, provided you have over 4K EMS of course.

However that means nothing I guess. If you ever checked your save playtimes, my played hours in Citadel (after reaching the beam) was more than 25 hours. After finish save dropped to 22 hours. Go figure.

You need 4000 EMS to have Shepard live, but ONLY if Anderson survives. If Anderson dies you need an EMS score of 5000.
Anderson always dies.

On my both playthroughs with 5200+ readiness and Destroy option my Shep lived.

Yeah I am aware of that but that means that ending never happened.

Anyway, I can't get 4000 EMS in any way so no. I don't have that... "perfect" ending.
Wait... why is it call "perfect ending" again...?
Did I mention that endings where terrible? No?

Earth saved
Sword saved
Hammer saved
Destroy Geth/EDI/Reapers/Mass Relay/Citadel. Though it also free the galaxy from the cycle and allow every species to evolve in their own way instead of choosing for them. I call it better than Merge just because of that.
Shepard live

My rating was 5800 (I did everything in the game, all side, N7 and Hub missions) and in all 3, Shep dies. I have never had where she is seen in the ending, even for a second. So I dunno what I am doing wrong. Also, even though she is 100% paragon and has all the blue choices with TIM, the very last one ( I assume needed to have him shoot himself) is never open for me...this is so frustrating.
5800 total millitary strength or effective millitary strength?

The only thing i can think of is did you try to persuade TIM to stop his nonsense whenever you spoke to him? Can you do the renegade choice? you should still be able to get it regardless of what persuade you choose.
