 You have the normal talk with Catalyst and he leaves you to chose an option. At this point you can choose to contact EDI and ask for help, telling her you would have to destroy the Mass Relays. Shepard asks if the info on Sanctuary could help, EDI reminds you that the research was destroyed BUT when in the Cerberus Headquarters she gathered some files but didn't analyse them as they didn't provide conclusive findings about the Reapers. EDI analyses the files and says that The Illusive Man did find a way to stop Reaper Forces but without a massive power supply was limited to small groups of ground forces and deemed 'Impractical', also he wanted to control them so didn't bother researching the signal further. Shepard gets angry/upset, but EDI reminds him/her that he is standing inside the galaxy's biggest power supply and sends the signal code, she can't help any more as she isn't there.
Shepard plays the signal for a moment and the Catalyst hears (he appears shocked/angry), he says it won't work but Shepard argues back. The Catalyst and Shepard argue for a while but after some time the Catalyst (very) begrudgingly gives in, telling Shep his/her plan won't work and the cycle will continue. He analyses the signal and tells Shepard it requires less power so won't overload the Relays but will only disable the Reapers temporarily as their core programming will find a way around it. Shepard calls Hackett and explains to him, they both agree they would rather go down fighting than doom millions of lives. Shepard tells the Catalyst to launch the signal.
The catalyst fizzles away and the room goes dark, the platform Shepard is standing on lowers and he/she is once again looking out at Earth and the ships fighting in front of it. We see extremely large numbers of Reapers, it appears the majority have left all of the other planets and came to Earth to battle. In a similar fashion to the other endings the Citadel opens up and fires a beam, but is not destroyed. Also, the beam is white and looks less powerful than those in the other endings, as it requires less power. The camera pans out to a view of the whole galaxy, white beams spread across however there are not any explosions as Mass Relays are not destroyed. The camera returns to our solar system and we see the Reapers stop firing, they seem to float and drift slightly. We hear Hacket order a full on attack, the combined Ships of the species' you have gathered attack and begin to destroy the majority of Reaper ships. We see groups of aliens bringing down smaller numbers of Reapers on their own home worlds i.e. Thessia/Palaven/Rannoch/Tuchunka etc... After seeing this for a short time approximately 10% of the Reaper forces reactivate around earth, the ships carry on attacking and some get destroyed. This is when you see all of your war assets in action, such as Geth Ships, Destiny Ascension etc... After some battle we see the last of the Reapers, Harbinger fall to a final blast from the Normandy (Although many other ships have attacked it). Finally the camera pans back to one final shot of Shepard looking out over Earth with the sound of many cheering troops coming over the comm channels. 
IMPORTANT: If you do not have enough readiness, you're forces will be decimated and you will see Shep bleed out looking over Earth.
That would be the end of the 'Story', the credits would play but that wouldn't be the end... Any number of things could happen based on previous choices. Some that I thought of are Shepard at a reward ceremony/Shepard on Rannoch with Tali if you romanced her or on Thessia with Liara (Basically with a loved one). After all this is over we see a quick convo between Shep and Hackett, Hackett says that there are a few Stragglers left but that they can be destroyed, we beat them.
