- Something like Assault mode from the original Unreal Tournament would be most welcome.

Maybe if they had no shields but regenerated health quickly. they'd be squishy but have good survivability if you used hit and run tactics, and a gnarly heavy melee were they jump onto an enemy and start going crazy with claws and teeth.
I'd also love to see some new classes, they would have to be balanced but still flavourful maybe the merc bands from ME2?
Krogan blood pack, Salarian Eclipse and Batarian Blue suns
blood pack could even have a flame thrower instead of the Cobra, it'd be you get 1 use per cobra round you had but it'd burn for say 5 seconds doing roughly the same DPS as a cobra does in 1 shot.
probably wouldn't be as good as the cobra but it'd be cool.
and maybe a Vorcha infiltrator that focuses on melee, tac cloak to get close an melee things, summon a Varen instead of a combat drone and maybe inferno grenades...
also customizable armour would be awesome, it would have to be purely cosmetic but it'd be awesome, i want my Krogan to look more like Wrex and less like Grunt.
as for levels i'd like to see one like the citadel from ME1 as in a kinda cylindrical level.
a sorta straight tunnel type thing with a sub level to give us ramps to defend but if you run up the side enough the enemy could be shooting you from directly above!

fight your way to this point and a super brute comes out.
beat him and keep going until super Banshee comes out
then maybe adds fight with protect this area against waves of husks
then boss fight against Maruder Shields.

Some ideas:
Generic Cerberus Base (since I'm uncreative) - You go through a Cerberus base and have to fight your way through it, along the way there are objectives to do. Like send a player up a lift to a control room to turn on a switch. There would be enemies in the control room. Whilst they are doing this you must fight off some enemies.
Other objectives too, like using turrets, looking for a key, and sometimes there might be a wave of enemies and you have to set up where you want turrets to be situated, and mines and other defensive positions for players to set themselves up.
Also a game mode where you fight waves but the waves are like onslaught waves. You are stuck in one area and there is one big door where the baddies come from and you have to keep shooting them else they will overrun your position. There is time between waves to place explosives, barriers and to grab ammo.
Also this is a really good suggestion for the multiplayer team:
Make it so there is a random chance that in the extraction part of a game instead of getting to the extraction point you might have to fight a boss instead. Like an extra powerful Brute, or three Banshees, or some sort of Kai Leng type super phantom that is mixed with a bunch of Cerberus enemies.

Your team has to board a Reaper Dreadnaught and do a number of hacking objectives to simulate breaking down its shielding in reactor areas etc followed buy placing explosive charges in key areas. Make it a shorter match but more intense, start people off at wave five style enemys for the challenge level and incorperate a hard cap time limit: finish before you are indoctrinated...
You could also do it on a Cerberus space station or Geth Dreadnaught but the Reapers kind own the indoctrination angle.... maybe the Geth timelimit could be them initiating an Oxygen purge...
I like the idea of something witha real feeling of urgency that has to foster teamwork and not reward solo snipers and lolguards running around ignoring objectives.

Speaking of customizability, more of it would be wonderful. Possible differing sets of armor or armor pieces, etc.

-Collector ship (I always loved the hive-like hexagonal design)
-Luna, Mars or London? (Someplace in the Sol system)
-Thessia, Tuchanka ruins or Citidel (someplace with beautiful architecture AND greenery)
Races:
-Geth (Infiltrator & Engineer)
-Prothean (Adept & Vangaurd)
-Batarian (Soldier & Sentinel)
Enemies:
-Mercenaries (infantry, mechs, sentinels, engineers with attack drones, krogan heavies and maybe a grenadier enemy)
-Collectors (a lot of biotic attacks like shockwave/warp/slow moving missiles)
Modes:
-Defeat a Reaper as a unit
-Defeat a Thresher Maw as a unit
-Co-op story missions/basic "hallway map" missions
Equipment:
-Armor/helm customization (at the very least deciding which of the available suits to use)
-No more class cards after customization is capped and/or experience is capped
-Actual skin color choices (true blue asari, black and white turian, etc)

The leaked DLC image shows Geth Inlf, Geth Engineer Batarian Soldier and Sentinel, it was mentioned that there would be protheans too so you're right on the money there. The image was from the store and showed the geths and batarians cards like when you get a new race/class from a pack so it seemed pretty convincing. Protheans and other contents (single player) of "The Truth" could just be trolling cos no image proof was provided.
