As I understand it, you get it via a random encounter, but I've finished the game twice and so far I have never seen it 'drop', or even where to go to get it, any hints/explainations?

It's part of the Warden's Keep DLC. There doesn't seem to be any criteria for getting the random encounter, I've had it at the beginning of the game, near the end of the game, and gone the whole game without ever seeing it. Just keep crossing your fingers, I suppose.
