There's a "strength modifier" listed next to a lot of equipment, but I don't see any difference in strength as a result of equipping it. What does it mean?

It means your strength bonus to damage is multiplied by this amount with this weapon.

No it isnt.
If you mean on shields, I think that's the damage modifier for the shield bashing skills.
