This is unbelievable.  It must have been due to a bug.  Right when I arrived at Vigil's Keep, the game jumped right to where I fight the Withered.  Therefore, Anders and Oghren weren't in the party (it still went through their speeches though when Alistair and the others arrived).  When we got into the throne room, Varel said that if Mhairi survives, there will be two Wardens.  She was essentially a companion.  I'd bet that, if I never do the Joining on any of the others (Nathanial, Velanna, and Sigrun) that I'll be able to keep her as a companion.  Wow! 

I just noticed that you can never de-select her.  If you do, she says "Andraste's Blood!", and is never re-selectable.  I also noticed that Amaranthine doesn't show up on the map until after the Joining is over.

My favorite Awakening bug was when the electric dragon threw me so far that I landed outside the arena and was locked out for the rest of the battle. Only happened once.

The most interesting Origins bug I experienced (and only once) was that somehow, at the shore of lake Calenhad, I ended up being able to walk on water like Jesus and walked out into a whole different universe. Very Twilight Zone. Has never happened again.
