what sustained abilities should a tank have on at all times?

That depends entirely on the situation. If you're surrounded by mobs, then threaten might be a good idea. If you pick tiny fights, like two to three mobs at a time, you don't even need shield wall, you could use shield defense. Or if you fight one mob with a dozen archers on your ass, you should use shield cover. If you're the one not tanking, however, I suggest shield wall and threaten.

Yeah I'm the one tankin' hehe. Still in Ostagar but I figured better to get schooled now so I'm not stumbling around later.

Shield Wall - get it soon as - you don't need anything else turned on, as long as you have 'Taunt' to use.

I have the tank 'tactic' to turn on 'precise attack' when stam/mana < 10%.

If you keep a small arsenal of immobilizing abilities within your party (shield bash, pummel, pommel strike, dirty fighting, winter's grasp, glyph or paralysis, etc.) you will find all those shield stances to be an absolute waste. Simply pull a few people at a time and focus fire on one at a time as you crowd control the others. If you run into ranged, find cover, send one guy out with a bow and have him pull a few at a time.

If they can not see you and they are damaged, they WILL come.

Shield wall is definitely not a waste to have on at all times. At least in hard mode you will be in serious trouble if your tank is surrounded without the talents that prevent flanking. Also, there is a talent that makes you immune to knockdown with shield wall on, which is a must have IMO.
