That FemShep was a dark-skinned, different hair styled Mediterranean goddess (dead ringer for Specialist Traynor - go fig!), so it's not just an issue of slightly different assets. It's assuredly not working correctly.

Removing the face code from the save file will force the game to use the new "approximation" method. Unfortunately, this method does not produce a perfect replication of the original appearance. An in-progress application, the Lazarus Project, has resulted in some success in recreating imported FemSheps by copying specific facial data detals from a Mass Effect 2 save file into a Mass Effect 3 save file and by remapping the vertices from the former game into the latter (the latter game uses more vertex points, requiring some interpolation of data).
Work is ongoing, and male Shepards are not yet supported, but many FemShep players -- myself included -- report near-perfect replication of their original appearance.

Is there any possibility to get around the "i looked on the ME2 editor and imported my ME1 face but now my face code is completely wrong" bug? I already tried to reset the code on my PC but I don't manage to export my savegame... And for time reasons "replay ME2 one more time without looking at the editor" is not a solution >.<
The Gibbed save editor for Mass Effect 2 may be used to delete the face code from a Mass Effect 2 save file.

Since Chris finally acknowledges this as a bug, I'm reproducing the face import bug thread on each of the tech support forums. I'll add a separate one, but here's the original…
Link to original locked discussion, last 20 pages or so have useful info about me3-lazarus
UPDATE: woodbyte's Lazarus editor is getting close to a reasonably simple fix! See below. Also, male examples would be welcome! We only have a few, and each one should help nail the problems down.
THE 1.02 PATCH DOES NOT SOLVE THESE ISSUES.
Fix Attempts and Analysis
- woodbyte's ‘me3-lazarus‘ editor is getting close to general use. Please note it's only working for female characters currently, and you can only edit PC or Xbox saves. To get the best results, you
should use the current code and follow these steps (or see Pheonix52gx's blog for illustrated/maybe more up to date instructions):- For more info, see the project page and/or look over the last few pages of the thread – or just ask a question!
- First, download the zipped build and extract it sowhewhere: http://code.google.c...zarus-0-1-1.zip
- Next, download (right-click -> Save As) the newer EXE file from r18 : http://me3-lazarus.g...arusProject.exe
- Also download the newest map from r17 : http://me3-lazarus.g...lod_mapping.xml.
- Replace the .exe and .xml files in the folder where you unzipped the first file with the ones you downloaded above.
- Start up the EXE. Open your ME2 save on the left, and your import save file from ME3.
- Optionally check the Scalar and Vector tickboxes. They should produce better match for skin tone, hair colour and so on.
- All you (should) need to do here is click on the Save button. The preview images look different, don't worry about that.
You should now have a new save file (numbered 1000 by default) that you
can load in ME3 to see how it looks. Remember that the squad screen
will probably still not look right, so look at gameplay and
dialogue/cutscene animation.- If the result isn't satisfactory, grab a screenshot and post it in the thread so we can try to figure out what's wrong.
- For more info, see the project page and/or look over the last few pages of the thread – or just ask a question!

Original Issue
Because of the changes in the head meshes, shaders and textures in ME3, it appears that some ME2 faces are impossible to import accurately. The import process results in an incorrect face, and in most cases it's not possible to get it correct by tweaking it in the CC. This problem seems to mainly affect female faces, but some males also suffer from it.
Even the default female face from ME2 is impossible to get in ME3.
There's a bunch of discussion about the problem further down, but I'll collect examples of these problems here, with pictures and face codes where provided. (There are several more examples in the thread itself, I may
not have all here on the front page.)
Some – but not all – of the remaining problems can be resolved by deleting the face code from your ME2 save file prior to importing. If that doesn't work, a more extensive surgery requires manually copying LOD data from a ME2 save into a ME3 save. Even this still leaves some incompatible cases. Hopefully, though, these heuristics can be added to a future version of the importer…until a proper fix is released.
Images Illustrating the Problem
To start, here's my own Eevy Shepard (I can provide face code if someone wants it).
The images are from ME, ME2, and ME3, and from left to right: Squad screen, dialogue animation, and normal gameplay.

The ME3 one is clearly a different 'person'.

Now, to demonstrate that this is, in fact, fixable and not a permanent problem with the new engine's "lighting and shaders:"

The patched ME3 character looks terrible, the fan fixed version looks identical to the ME2 character.

Oddly enough, the 'imported' face looks exactly like the last incorrect male that Mingolo posted, even though my character in ME1/ME2 looks a lot different from the red-haired source image directly above it.

I was able to recreate one of my ME1 Shepards after an import through ME3's customization. But will not go through that with two more characters.
This stems from a bug in ME2 when importing a ME1 face and going to custom faces and then exiting and using the original imported face.
BioWare should have had that fixed BEFORE launch. They need better QA and they luck in keeping any customers for whatever game they use to further exploit gamers.

If you have this problem, get yourself the ME2 Gibbed save editor.
Export your save games to the PC: (basically explained here)
social.bioware.com/forums/forum/1/topic/323/index/9247811/3#9249327
- [list=1]
- Make a Backup of your save games.
- Open Gibbed, load the save game you are planning to import in ME3.
- Go to the "Raw" Tab, expand "Player".
- Scroll down to "Face Code".
- Delete the existing Face code.
- Save (You should not overwrite the existing save. Example: when the save you loaded into Gibbed is Save_0053 rename it to Save_0054 in the save dialog.)
Import your save games back to the xbox

As far as I remember the Instruction, you don't need to log'd on to the Modio servers to extract your save game from the Xbox.
