Often now when I play as Vanguard I get a few rounds in and then the world starts to shake around the character. The problem progresses to the point the the character is vibrating a foot up and down, only barely capable of normal movement, gun doesn't reliably fire, abilities are more difficult to trigger. Then the issue continues to worsen: I'm unable to move at all without using Charge or rolling, gun doesn't fire at all, and if there's enough time left in the match my character will either start falling through the floor or, in one instance so far, shuddering all the way up to the top of the map. Total freeze generally follows.

So far this has only happened when I play as a Vanguard, though freezing and sudden disconnect followed by being unable to connect or purchase items also occurs regardless of class.

I notice about this happens when I am dying and will be dead, and at the same time I activated Biotic Charge. And the glitch will happen if you died and you see the animation of the Biotic Charge after getting killed.

I read previous post about this glitch saying about the latency of the host and the players. That could be the problem, but I'm can't confirm it.

I tried to continue playing the game, but you must only use your weapons to kill. If not you will end up flying or falling, and that is when you'll have no control at all.

It's not a good solution, but beats having to leave the game when you got high scores, right?
