So I did a quick search but did not come up with anything.

Basically my issue happened when talking to someone on the Citadel after the attack and when I started a conversation with them the model just disappeared but I was still able to him. I can not remember their name but they are in c-sec office on the couch to the left through the door.

Something similar happened on Rannoch but with the rock that is picked up

Both these also happened to me on the PC version, with the Alliance Captain at C-Sec and the rock. Also my friend said the same thing happened in the PS3 version.
Also, Khalisah al-Jilani was invisible to me, though she was talking to me; until I saved and reloaded the game.
