I spent around 5 hours spread in 4-5 days to make this work, and i hope this info might help someone who got cought desperately as i did :) More on that later. Also check the bottom of the post for some PlotID bool values to use with Gibbed save editor for those who are desperate.

Here are the things you have to do to make her live in the end of Horizon Sanctuary mission:
  •  Make her fully loyal in ME2. edit It is reported that you do not have to make her fully loyal, but you must complete her loyality mission.
  •  Make her survive Suicidal Mission (obviously).
  •  If you romanced her at any point, do not break up with her. edit It is reported that you can break up with her in ME2, just not in ME3. It is also rumored that you can break up with her in the hotel room (meeting 3) but you cant do that during first meeting - this will kill her definitely.
  •  Meet her at the Docs in the Citadel BEFORE Cerebrus attack on the Citadel. Talk to her nicely and ask how you might help.
  •  Read Kai Leng dossier on your (or Shadow Brokers, not sure wich) terminal after you encounter him before talking to MIranda again. edit reported as unnecessary.
  •  Talk to Miranda through Spectre terminal on Citadel, and warn her about Kai Leng (make sure she responds with "I will make myself ready" or something)
  •  Meet her on the Citadel later, and when asked for Alliance resources for some operation, give them to her.
  •  At the end of the Horizon, use Paragon/Renegade option while talking to her father.
This should keep her alive. If you miss any of the above mentioned, she dies. I did miss the first meeting at the Docs, and was stuck with her dead. So i did some research for my situation and while helping some people. Here are some Plot IDs you might find helping.

1. First meeting in the Citadel with positive outcome (ask her about her situation and promise to help).

20182 True
20183 True
20741 True
22698 True

2. Second meeting via Specters terminal - we do not have the data yet, so if you do, please share.
3. Third meeting in person on the Citadel - give her the resources she asks

21872 True
21873 False

4. Miranda BreakUp: finally, thanks to Jereee, we have PlotIDs for breaking up with her in ME3.
If you did break up and you have changed your mind and want to keep Miranda alive, set

21751 False

Hope this helps :) Mad props to @gibbed for his tool and @rsanchezsaez for a working hack i used to export and compare plot bool variables, BossBast1 and Jereee for providing me saves at turning points.

I could be wrong but I don't recall reading any dossier about Kai Leng and she survived my game being loyal, non romanced.

Just be nice to her in ME2 and make sure she survives. Go back to the citadel after every mission, read your mail regularly. Always check the citadel mini-map.

- loyal
- survives ME2

Make to sure to....

- read up on Kai Leng after you meet him
- talk to Anderson after you meet Kai Leng
- read e-mail from Anderson
*not sure if Liara's file on him counts, read it just in case... if she has it
- after you read about Kai Leng from Anderson...
- go back to the citadel
- go to spectre office and "call" Miranda
- meet her in the citadel apartments to catch up in between all of this

that's it.

Same here. Giving her what she wants and warning her seems to be enough. Then just make paragon/renegade choices at the cutscene on Horizon.

again, you have to meet her all 3 times on Citadel, 2 in person 1 via specter terminal. If you doubt this (and miss the first meeting) you will end up with save editor lurking for plot bool ids for that first meeting, as i did :)

I didn't realize it was that hard to save her, she didn't die in my main playthru, and never thought about her dying TBH. Might have to skip one of those to see the scene in my current playthru.

The only three people I lost in the entire game was Thane, Mordin and Legion and that's with full 100% all squad members from previous titles alive (except Kaiden due to picking Ashley on Virmire). I did not need a guide but instead went on instinct what I would do in those situations and my character. I guess I made the right choices without need for help. I want to know is it possible to save Thane though but I believe it is the case he cannot be saved in Mass Effect 3.

I didn't realise it was this complicated to keep her alive... she survived my game, in which she was loyal but un-romanced in ME2, I met with her 3 times on the Citadel and gave her access to Alliance databases. Then charm/intimidate option in the Sanctuary mission.

Dont break up with her! (you can still romance someone else, just dont tell miranda to F off)

I did everything right and she died in my first game. I was pretty pissed.

I know my shep mentioned him to anderson who sent me an email with a lot of information about Kai Leng. Think it was anderson.

Yes it is possible to safe Thane. And yes its a plot continuation from ME1 - Kirrahe have to survive. You can also save Mordin but you cant save Legion - he either goes by his own will, or is killed by Tali.

btw i also did everything correct "trusting my instincts". missing one meeting with her wich i thought was optional - you meet her after the Cerberus Citadel attack anyway - hit my insticts pretty hard :)

I didn't even know Miranda could die in ME3.
Now, if you figure out a way to save Legion, that's some info I'd like. If everyone else can be saved, maybe there's some insanely obscure way to save him too.

This.

Well, I let Samara die, but that was intentional - I very mindfully did not use the Paragon interrupt which I presume saves her life. Just seemed fitting, because for all her wondrous cleavage I feel that that character always sucked, and frankly having her "violate the Justicar code" in the name of saving her not-bat****-insane daughter would have been far more intriguing character development than the cop-out suicide bull. Not gonna lie, I chuckled and moved on.

I was actually surprised to hear that the possibility to save Mordin exists; I guess that requires you to not opt to give the Krogans their cure? As for Legion and Thane, I find it hard to believe scenarios exist where either survive their respective circumstances.

I spaced the e-mail, yes I did get that in my first playthrough as well. It was Anderson as I just got said e-mail it happens after you talk to him and Hackett in the comm room after the Citadel attack.

Why would you want to save that **** for? Nothing but a cheerleader outfit prancing, Cerberus ****. I'm Aussie myself and I still don't like her.

I did all of that, but refused to give her alliance intel and she died. So you have to do everything on the list.

Yes.

Actually, BW really wanted her dead. I liked her ME2, but she'sa worthless character in ME3, she's better dead.

3 is not true. I romanced her and then broke it off before the suicide mission, and she survived in ME3

I never romanced her in ME2 (was FemShep), and warned her about Kai Leng. She lived.
