The only way for Miranda to survive Sanctuary is if she's loyal and all the other requirements are met.

No, I'm talking about the Sanctuary Mission. My first import didn't have her loyal, because of the Jack and Miranda feud. In ME3, she still lived after Sanctuary. I did everything required and she still lived. This was on PS3.
