I went into the game thinking Shep was probably going to die, and thinking that for the entire game. And I was okay with it.
But thing is, for an RPG of this scale, there shouldn't be only [i]one[i] ending. Maybe Shep dies in the majority of the ending, but there should also be options where Shep lives, depending on choices that you made.
Either way, all endings should make sense. Sadly, what we were given didn't.
Would you be fine with Shepard dying?

I'm ok with an Ultimate Sacrifice ending but I would prefer if Shepard and his/her LI are both going to die that it happens together without any weird teleportation or what not. But some closure after the fact would be nice, maybe a montage showing the state of the galaxy, maybe a funeral/memorial, something that makes it definitive.

Many people is even of the idea of having a REAPERS WIN ending, because it matters... if you don't work hard to build your forces through game decisions you should not come out victorious... In the same way that if you do everything on your power to prepare, you should get an ending with Shepard & Co surviving. And many others with various degrees of sacrifice. Only Shepard dying, Shepard and most of his team, etc.
Would I pick the ending on which Shepard dies if I had the option of one where Shepard lives? YES. In fact I would play multiple times to experience all ranges of endings, the same way I played multiple times to get all possible combinations of Dragon Age Ending. The funeral scene after going for sacrifice was very rewarding. But to me the best option was sacrificing Loghain, because you helped him to get redemption.

I think it's fair to assume the ratio in the various endings has to be in favour of Shepard's sacrifice.
I was expecting to sacrifice him, or the Earth for what i knew.
Heck i wanted to sacrifice Vega, Ashley or Javik. Didn't hate the third, but he seemed pretty willing to give his life to further the cause.
That said, i'd like to see the awesome-happy ending with Shepard alive, not under a pile of rubble on the Earth, contradicting 2-3 things at least with a single scene.

As it's been said, if Shepard's death is an option like a Warden sacrifice being a choice in DAO was, I most surely could work it into the story of one of my Renegade Shepards no problem. I'd rather the choice than it being forced on me regardless of what happens with the higher EMS rating (PS that breath scene really could have been a bit more dramatic than a quick two second inhale after all that work to get it :| ). That's not really my huge problem with the endings though, but that's a whole different story completely.

I think Shepard should be able to survive or die depending on the play through. Shepard should be able to survive and have a hero's ending (unite with LI and crew with relays and Citadel operational). But I think is should be very difficult to achieve. Maybe only on insanity with a maxed leveled Shepard. They should make you really fight for that ending. All other ending should be something short of that with a combination of life or death for Shepard.

I was expecting Shepard would die. Heck her death from the "Destroy" ending was pitch-perfect for me, just the context around it that bugged me.

Id like options with different outcomes as the ending instead of being giving 3 options all using the redundant boring cliche "Self sacrifice" ending, but bioware got lazy and counldnt think of a better way to end it.
