This is how my "perfect" ending would be if BioWare wanted to fix a couple of things. Please note, this means tweaking plot holes and fixing continuity issues, while adding a little bit of player choice. Overall, the ending DOES NOT CHANGE. IMO, that's BioWare "Holding the line" between what the writers created and what the fans needed.

FIRST - Everything gets left the same, but we get to ask some of those questions we didn't need to know...like how long they've been reaping.
Second, when the kid says synthetics and organics can't live together, Shepard makes a very Shepard like comment of, Wanna bet? Instead of, Maybe.
Third, when finding out he/she can control the Reapers, he/she doesn't say "TIM was right, after all"

This is based on Shepard choosing Destroy, as it is the best ending.

*The Mass Effect relays receive the blast from the weapon, but ARE NOT destroyed. This continuity error has bungled too minds. Please, BioWare, fix it. NOTE: This also fixes the HUH? factor of all the races stuck in Sol.

*Joker is outrunning the shockwave, but not in the middle of a Mass Effect jump. Nor, does he crash on some paradise planet.

*If Shepard chooses to upgrade the Geth, they are now considered "biological, and survive.

*If Shepard properly guided EDI to being "Human", she also survives.

*The explosion doesn't blow up the Citidel. (That's just me loving it)

This last part is the easiest to do. It requires recycling some of Seth Green's dialogue and overlaying the audio with existing video.

Cinematic runing across rubble to show N7 dogtags. "Commander, come in. Commander? Do you read me?" Shepard takes a breath. Credits roll. THAT'S Jenga.

For those who want an epilogue, and I'm with you, it can be done in a series of stills during the final credits with a Keith David voice over, or even just text.
