Diablo I and II (LAN parties, yay)
It really is a shame that they have delayed Diablo III to the point that even people that WERE hard core fans no long care.

Kotor 2 - 10 or more, not as cool as 1
Jade Empire 10 - 20 times
Oblivion, put it this way, six months waiting for surgery made me put a lot of time into that.
Dragon Age - on my 3rd right now - would be 4th if I didn't miss Leliana and Sten in Lothering.
Civilization Series - that has a replay value in years, not just in times through. Refuse to admit that number.
Galaga, if I had saved up the 20cent pieces I spent on that, I would have had a thousand dollars by the end of High School.

Uncharted 2. The game doesn't even have really all that much replay value. It's just such an amzing game that I want to play through it again, and again.
Demon's Souls. It's an amazing game with more replay value than it looks like on the surface. It's like Monster Hunter in that way. I summon some Souls to play co-op, and we play. Or I enter other people's games as a villain and try and frustrate them. It's very unique in that.
Sacred 2 Fallen Angel. God, this game is outstanding when it comes to replay value. Loot grinding at it's finest. And I've never experienced a game with such deep stat building. It's actually TOO deep for a novice. You can really break a character. But once you learn the basics of building each class, it's great. IMO the game's actually more fitting for console play. I think console player will like it more after playing Dragon Age, if they looking for a similar game play experience just much less story driven with online loot grinding.
Killzone 2. I just can't get enough of Killzone 2 online. And I don't even see any FPS coming in the next year that will replace that experience for me. Maybe Alien vs Predator, but after seeing that game's online, it looks ultra repetetive and unbalanced.
Oblivion....Obviously. I didn't even move on to another RPG until Sacred 2 came out on PS3. I still go back from time to time
Unreal Tournament 3. It's the first console FPS with true mod support. Truly infinite replayability. I still download a map or 2 a week along with various other mods like characters and muts.
Monster Hunter. My favorite ARPG of all time. I only stopped playing because the servers were shut down. Between my 3 characters I had 1600 (sixteen hundred) hours just online.
When I wasn't playing Monster Hunter at the time, I was playing Champions of Norrath Return to Arms. LOVED that game. My father still plays it.
I can't wait for Heavy Rain when it comes to replayability. It seems like it's going to be the epitome of the term. I looks to be a game you could literally play 20 times and have a different experience every time. Highly anticipated.
And also Gran Turismo 5. When it comes to replayability, I can't see any racing game have more. Not only is it a complete GT game, but also a NASCAR game, and a WRC game. 3 racing games in one.....yea.....I'll be playing that for YEARS.
Sounds like we have pretty similar tastes, except for NASCAR
UNcharted 2 is still sitting on my shelf, IDK i'm kinda burned out on Cover shooters and cutscene heavy games

Never really got into NWN, because it lacked party combat. Think I only played NWN2 once, though I have been thinking about giving it another go. Can't get back into Oblivion. I found it to be a really dull and uninteresting game once you complete it once, more so than most other games. Same with Fallout 3. The replayability value of those games are much lower than I would have expected.
The Unreal series (1 and 2) is another set of really old games I like to jump into for nostalgia's sake (I played tournament-level Unreal CTF for several years back in Europe).
Come to think of it, I think this list encompasses the handful of games I brought with me to the states when I moved here a few years ago.

Think of Chess, its timeless as far as replay value is concerned.
Dragon Age Origin, is more story driven than anything else - its kind of like a movie. I think once the player knows the story, the replay value greatly diminishes.

MP games "don't count" as they are designed to be replayed from the start. We can automatically assume WoW< EQ, CHess, Checkers, etc...

In terms of number of hours logged:
1 Hearts of Iron II (with the CORE mod)
2 Europa Universalis II & III
3 Civ series
4 Morrowind
I've never replayed a linear RPG and have only made it to the end of a very few- Mass Efffect was probably the first since FFIII. I suspect DA:O will eventually make that small list, but we shall see.

Knights of the Old Republic - Countless times.
Neverwinter Nights - Still has an incredible community with lots of diverse persistent worlds.
BlazBlue: Calamity Trigger - 2D fighting goodness. Online competition is superb.
Besides those rather obvious choices, I can say that I still can't get enough of Super Mario platformers, the Zelda series, Super Metroid, and Gradius.

Currently on my PC. Civ4, WoW, Oblivion, Gal Civ.
Played alot of games, but those are the ones I went back and played time and again.
Only played BG once through I think. Managed to miss NWN despite being told repeatedly to pick it up.

Mass Effect - Somewhere around 3-5.
Morrowind, Oblivion and Fallout 3 - Not sure on the number of replays but definitely spent a ton of time on them.
I never could get into NWN, not sure why. Thinking of trying NWN2.
Saints Row 1 & 2 - Played them both a few times, same with most of the GTA series.
Fable and Fable 2 - Quite a few times through both since they're pretty short.
SW - The Force Unleashed.
DAO - Obviously.
And since people are listing MMOs...the one game that I've consistently played for over 4 1/2 years - WoW. Currently playing some LOTRO right now though.
